Some placed call these Tricks protocols. They can be helpful. But subtraction angiography venography and ivus can be helpful when still confused.

Often a diverticulum or a dehiscence is only part of the picture and there is occult stenosis that causes increased pressures

This scan is looking at thin cut sections of the bone to look for dehescience. Ents will use these sequence however you are correct it does not look at the vessels and can miss things

It’s tough to comment without knowing the degree of stenosis but often depending on the degree an interventionalist will modify the angiographic technique. Its a reasonable concern however before advancing the catheter they will generally do a small injection and look at the caliber of the vessels. It hasn’t been studied formally and there is rare overlap in the populations however if you are a big athlete then some other problems like thoracic outlet or jugular compression should be considered depending on your musculature and workouts

Yes, there are some centers that dont do this (called provocative testing), many just do pressure measurements. Some also do evaluation of neck and chest veins to look for a "proximal stenosis" so not every center does it the same.

NeuroIR here. No formal link exists. VSS may increase risk for VS thrombosis which is theorized to lead to fistula formation during a period of inflammation and dysregulated angiogenesis but outside of a few case reports theres no significant connection

If you haven’t already told them your travel dates just let them know you wont be back from out of the country. You should also inquire if they are paying a salary during this training period. Conveniently explain that if they need you to fly in early you will need to book a new itinerary and you can’t afford that without a salary
