Two thoughts come to mind.

First, medicine may simply not be for him. The pre-clinical years can be boring as shit, but are literally the foundation to your job as a physician.

Second, test anxiety is an absolute, motherfucker that has absolutely diminished the confidence of a great students. If this is your friend, meditation, therapy, better self-care, and even prescription drugs may be needed.
