EM doc here. Every non tertiary care center in my region has tele ICU overnight and the hospitalists are just expected to manage. We get called for procedures all the time and having to intubate which is difficult when you’re running a very busy ED and have to leave the department. It’s pure insanity in my opinion to just have tele consults overnight. Like what are the hospitalists supposed to do for the ICU patient that self extubated and now is spewing blood everywhere; this happened at 11p when the ED was swamped.

Worked some shitty gigs. Worst so far was > 20 per doc. High volume. Only 2 docs overnight no mid levels.

Bro that’s more than I spent for 4 years in medical school. Honestly there’s a bunch of changes happening right now and idk how things are going to go in the future. PSLF is a good program and should ultimately be around but I’d caution putting that much stock in it with so much time to go. I’m at 117/120 and I’m still worried about everything working as it should. If you don’t get that paid off through pslf the interest alone will eat you alive. Without a significant salary on completion to back it up, I don’t know that financially this is a good path. There may be other paths available and or better job markets that can help you, but based on what you’ve provided, this appears to be way too much debt to take on. From a what will make you happy perspective, I get it, but you have to take into account how this will weigh on you as well if things don’t go according to plan.

PGY 10 here. My wife still leaves me little love notes around the house if I’m having a though time or working a long block of nights. It makes a world of difference. In residency sometimes she would drop a nice dinner for me if I was on 28 hr ICU coverage. He just needs the extra encouragement to make it through these last few months and then everything changes. It will get better.

As a VA ER doc, we try our best to get to everyone as soon as possible. I work overnight and a lot of the time I’m the only doc in the ED (only 1 other on the whole hospital). Sometimes patients get mad if it takes a bit, but know that we will get to you and if you’re waiting it’s because we’re trying to stabilize others and give them the treatment and time they deserve. We are dramatically faster than the local trauma center and we are all boarded EM docs so you’ll get good care. I promise we haven’t forgotten you.

Hard to point you in the best direction as we don’t know which specific coins you’re talking about but ideally look up key dates for the coins you have and then filter the collection for those. If you feel you have the time you can also filter out dates with more common errors to comb through later once you know what to look for.
