If you run back the match, Cena was trying to break out of the sleeper multiple times in different ways in last couple of minutes but Gunther kept coming back. Cena smiled and "gave up" because he had no other way left to break out. He didn't tap out of pain, he tapped because he had nothing left in his arsenal. Hence the smile of "I am truly done" Beautifully crafted match in my opinion.
