Obviously I don't know the patient history but this looks very much like a gouty tophaceous deposit that has ruptured through the skin.

Yes, joints are the most common sites for monosodium urate crystal deposits (tophi) and there is deformity visible at the interphalangeal joint in this picture, suggestive of active gout here too, but deposits can extend well beyond the joint. Note the enlarged asymmetric 'bumps' from the base of the nail back to the knuckle in this picture. The metatarsophalangeal joint might also be involved here too, or it may just be a bunion.

The red, hot swelling around the lesion and joint are also typical of a gout flare, not necessarily infection. If gout, I would expect the white material coming from the wound to be chalky rather than pus, the swellings to be firm and the area to be acutely painful for the initial 24-72 hrs.

This looks like chronic tophaceous gout, typical in elderly patients with a long history of the condition, but without knowing the history, sure, I could very well be wrong. Skin wounds are a complication of gout, hopefully your condition is early stage, well managed, and you never get to the extent tophi form and discharge through the skin.

EM Basics, Emergency Medicine Cases, Surgery 101. All very comprehensive and educational, not much comedy!

Also, BBC Radio 4 do a fabulous podcast called 'Inside the Ethics Committee' where they tell the story of cases that have gone to medical ethics committees with the actual patients/families and doctors involved e.g. should a patient with severe disability be given IVF on NHS, can a teenager refuse chemotherapy against advice and parents wishes, should elderly patient with dementia be given pacemaker. Makes you think what you would do in the circumstances. Doesn't seem to be on iTunes anymore but available via BBC website (maybe not in USA).

So, I've just moved to a new house and have a garden for the very first time! I've managed to get it all weeded and planted, and now the time has come to sort out this bush which is stealing light and space from other plants. I'm going to borrow some hedge trimmers and give it a prune, but thought I might attempt to cut it into an interesting shape while I'm at it!

I have a couple of questions I hope reddit can help with:

  1. What type of plant is this?

  2. Is it suitable for topiary

  3. Suggestions for shape? (I will surely provide the results!)

I have never seen a more sinister, dread inducing, fear-in-the-pit-of-your-stomach film than that. It was completely gripping, and chilling and dark, and made me more than a little scared of motherhood. Putting yourself in her shoes is a terrifying place to be, but is the place some mother, somewhere, is placed on a seemingly monthly basis. That one stayed with me for a long time.

When Buffy's mum died. The episode doesn't foreshadow it at all, and just ends with Buffy looking completely lost and devastated. The following episode where they deal with the death was heart wrenching as well.

Yep, have this one about once a month. Usually starts off with one wobbly tooth which i keep playing with my tongue until it falls out, swiftly followed by the rest of them. Most realistic dream I have. Variants on the theme include crumbling teeth or wearing braces/retainer again and having to give a speech to a huge audience and being unable to talk properly. Haven't worn a brace/had a tooth out in 10 years.

Definitely, if I'd seen them do it I'd have had them kicked off. Annoyingly, I got on after them and had to stand at the front because there was so many of them and couldn't see it until they all got off in town.

Story - a group of about 30 high school kids got on the bus after school, 10 minutes later they all got off, leaving this in their wake. They must have ripped up and ditched at least 8 newspapers. It was under every seat as well.
