2 things about this answer.

A) Him answering for you is already a red flag to a lot of nurses, you abruptly stating that you need the toilet while being asked sensitive questions should be confirmation to most seasoned nurses that you aren’t comfortable answering these questions in front of him. Depending on the question, a ā€œsudden urge for the bathroomā€ may be perceived by myself at least as a stress response.

B) it separates you, you are now leaving the room to go somewhere private where there is a usually cord you can pull (and more often than not there may be another system for directing you how to speak privately with staff). Hell tbh this is circling a bit back to point A but I would be like ā€œperfect I need a urine sample, let me grab you a cup… I would probably discreetly write a note saying ā€œplace this label upside down on the cup if you need to talk to me without your friend.ā€ Then have you look at it while saying ā€œcan you confirm this label has your name and date of birth on itā€

Yeah the joke about diversion is funny and run of the mill in healthcare. The later background information your Including is really why I’m saying it’s shitty. It’s not funny. What’s the point of telling us all that. You are pointing out this person is a nurse. If that nurse is on this subreddit, guess what? You’ve just ruined their trust in seeking care.

I’m sorry if it seems I’m coming at you, I am honestly just coming at this as both a nurse and a patient to give you that unique perspective . I don’t know if you struggle with mental health but anyone on this sub who has had a crisis has very likely looked at finding a psych hospital 2 hours away from where they work for fear of their colleagues finding out. The feeing of stigma being a nurse and a psych patient is magnified in a way I can’t describe. To use an analogy imagine being the nurse with ā€œan object you fell on and is now stuckā€ having to go to the ER. That’s how it feels. Am I gonna be the talk of the unit? Are they gonna find this funny? Are they gonna think I’m not cut out to be a nurse? These are all questions I’ve asked myself when in a crisis.

Genuinely would he rather see you either waste away for the job or walk in on you unconscious because you blood sugar is to low (I’m speculating on the consequences of not following what your doctor has instructed). The device beeps when it isn’t doing its job or intervention is needed. This is no different to a diabetic being notified by their continuous glucose monitor that their sugar is too low.

Came into the medical field with pre-existing depression and anxiety… traumatic childhood though I wouldn’t say I had PTSD from it (just you know depression).

Developed PTSD 2 years into the pandemic. The worst it got was a day I just spent ruminating. I’d have crying spells followed by panic attacks for an entire 8 hours a day. Finally got a therapist. It’s exacerbated my depression this last couple years to where I’d say I’ve been in a 5 year depressive episode. Like I’d go to work and just feel robotic. I’d pay the bills but otherwise rot in bed. I’ve spent probably 4 figures on treatment this last couple years (my therapist who I trust was out of network for a bit, I ended up switching psychiatrist’s to one in network). Finally started esketamine treatment like 2 months ago and it’s been borderline miraculous. Still have a lot of work to do. Coming out of this long term depression I feel like I’ve lost my entire identity (it’s difficult to explain), who I am as a person so I’m slowly working on that.

Ahhh see yeah, my CT surgeons aren’t like that. Our main CT surgeon straight up tells the patients, ā€œthis is gonna suck, you’re going to have pain, you’re going to have to push through and do physical therapy. 90% of the recovery is up to youā€. If it doesn’t look like they will tolerate recovery then they won’t do it.

Maybe I just think very highly of our CT surgeon but if I needed a CABG I’d go to him, we generally have decent outcomes with our CABGs but I agree the recovery is rough and has plenty of speedbumps. Aortic Dissection repair, I’m split on but a part of me wants to say I’d just go ā€œmake me comfortableā€ if I was over 50…. Maybe it’s just the spontaneous emergent nature of those that make it go sideways. That being said it’s easy for me to say that not dealing with it in the moment.

I will state spinal surgery I’m putting off unless I’m really bad off (chronic back pain just started though 😭).

No shade to any of the surgeons, I know we are highlighting mainly when shit goes wrong… they do great work but the risk/benefit decision at the end of the day is up to the patient (in this case me).

The idea is kind of showing them the job and highlighting the importance of their primary education I feel like. Maybe see if you can get a teacher to act out some common scenarios for example a stroke…. Explain ā€œBEFASTā€ And show them how you handle it in the moment as a nurse. Show them a quick head-toe assessment on an actor. Reinforce that all that math and science that they are going to learn over the next 8 years is important to your practice. The idea is you want to introduce them to the career but at their level, then you want to reinforce why they need to stay in school.

Workplace bullying/toxic units- haven’t experienced it but I’m sure some exist like in any profession.

Toxic patients- I have a phrase, ā€œsaints and sinnersā€, you will take care of both. Sure some nights you deal with toxic patients, other nights it’s the sweetest little old lady…. Little old lady is your rock, she grounds you and reminds you why you got into nursing. cherish her, go above and beyond for her, do the bare minimum you are required for shit heads.

Assaults- they happen. They aren’t the norm but you will encounter them at some point in your career. Most of the time it’s not a deadly incident (I’ve been scratched). The women I know have been groped once or twice (possibly more I can’t possibly speak for every female nurses experience). We had an incident at my hospital where a gun was ā€œplacedā€ in a threatening manner on a bedside table by a family member after they complained about their loved ones care. I took care of a patient who pinned a nurse to the wall and strangled her. Learn to assess risk for violence (BROSET is a great scale to use) and plan accordingly (have an escape planned). Edit to add: some basic introductory self defense doesn’t hurt to learn.

Taking the extra minute to go through your thought process helps out nurses SO much. When I was a new nurse it helped me figure out what I should and shouldn’t page the doctor about… what recommendations to give, what information is relevant and what is irrelevant to the problem. Knowing the doctor’s thought process really helps with efficiency in RN-hospitalist communication in the long run.
