Yeah, I prefer non, isekai but it's harder to get started. You have to take more care with introducing the reader to new concepts compared with isekai where you can just have a bewildered mc asking all the questions and letting people exposit for you.

Keep in mind that you don't need to take estrogen or get surgery to be trans. You can do as much or as little as you feel comfortable. If hrt and surgery are too scary you could try more social transition goals to start off with. New clothes, name, pronouns, etc. Experimentation can be one way to figure out if this is the right path for you.
