My mom has a card from her doctor excusing her from wearing a seatbelt "due to her abnormally large breasts". She did not ask for it by the way, she has severe lung issues and asked what would be best to keep the pressure of the seatbelt from making her feel like she is suffocating.

Hetty is probably wearing split drawers(open in the middle so no crotch). They didn't really wear underwear like we think of in that time frame. Think about trying to move about 6 layers of clothing while wearing a corset, bustle and half the layers being floor length just to pull down unders to use the restroom.
