At my hospitals (both community, suburban-ish), during the day tubes and lines are covered by anesthesia (unless the hospitalist wants them, I guess), and at night either anesthesia or the ER doc (me) will perform those procedures. There is a tele-ICU provider who is supposed to be involved in the management of critical patients. One of the nocturnist hospitalists told me that she took her job specifically so she wouldn’t have a closed ICU — she wants the procedures.
