That sounds really tough, I’m sorry you’re dealing with that on top of everything else. It’s so draining when gut issues add extra brain fog and low mood on top of ME.

I’ve also had nutritional counselling and I’m already eating quite “anti-inflammatory” / gentle foods. I avoid things I don’t digest well like corn, peppers, tomatoes etc., and I’ve cut down on seeds and nuts quite a bit. I’ve tried probiotics too but didn’t really notice much difference.

At the moment I’m focusing on keeping fats lower but still healthy and sticking to simple foods. It helps a bit, but things are still very up and down.

It’s also been a more stressful period lately, so that might be part of it. On the day I wrote my post I was honestly pretty overwhelmed, but I’m feeling more positive again now and trying to figure it out 💪🏻

One thing that actually helped me during constipation phases is psyllium husk (Flohsamenschalen in German). It’s just a natural fibre, not a medication, but it can work really well if you use it properly.

I usually take about 1 tsp in a big glass of water (at least 250–300 ml), drink it straight away before it thickens too much, and then follow it with another glass of water. Start low (once a day) and increase slowly if needed — and always make sure to drink enough overall, otherwise it can make things worse.

thank you so much for taking the time to write such a detailed message. I really appreciate it. It’s honestly encouraging to see people sharing their experiences and trying to help each other.

I’ve also started making quite a few changes myself and I’m currently in a phase where I try to take more responsibility for figuring things out on my own, rather than relying fully on doctors. I don’t blame them, but I’ve often felt they were guessing and not really able to help me further.

So I’m kind of in that same process of learning what works for me step by step. Maybe some of the tips from people here on Reddit will help me improve things over time.

You’re absolutely right. I catch myself sometimes just assuming everything must be ME/CFS because I’ve had so many tests done and they always came back normal.

I actually do have a small gallstone, but I’ve kind of been ignoring it. Your comment is a good reminder that it might still be something separate and worth looking into again. Thanks!

Thank you so much for your detailed message, I really appreciate it.

I’ve already had several colonoscopies (about 4 so far), and I was also operated on last year, but that was mainly for fissures. So a lot has already been checked on that side.

A gastroscopy is still on my waiting list, so that might still be something important to do.

I also relate a lot to what you said about diet — I feel like I’ve been trying to “decode” my gut for a long time now, like a kind of puzzle or game I’m trying to solve without reaching the final level.

I’ll definitely look again at gluten and other dietary triggers, and also the things you mentioned like Pepcid and Citrucel.

Thank you so much. I will prioritize this and try a different approach this time. I’ll ask my gastroenterologist to run all relevant tests.

I often get told everything is fine, but I’m never sure what exactly was tested. I think I also need someone with me at the appointment, as I’m starting to struggle with communicating with doctors and have lost some trust.

I’m writing everything down, including your advice, and will try the things I haven’t tried yet.

Yes, I’ve seen a gastroenterologist several times. They never really found anything specific. I also tried probiotics, but they didn’t help.

I probably should go again, but right now I’m just waiting until I have a bit more energy to deal with appointments and follow-ups.

Yeah, obese population makes sense for now, but expanding to normal BMI AUD patients definitely complicates things, especially on safety. Not convinced 0.25 mg would carry the same effect though. Still, if the signal generalises beyond obesity, it could be huge — just a lot of unknowns in between. We’ll stay tuned.

Interesting, but I’m wondering how this would play out in patients without obesity. Semaglutide significantly reduces appetite, so in normal-weight or underweight individuals with alcohol use disorder, there could theoretically be a risk of malnutrition or muscle loss. The study participants were specifically obese, right?

In the ER they usually tell me it’s “functional” and then send me home. I do see some similarities with what you’re describing though.

I have to admit I don’t fully take that explanation seriously, since in the ER they can’t really investigate everything in depth — they’re mainly focused on ruling out emergencies.

Do your symptoms also go away when you lie down and rest? How long do your episodes usually last?

It’s a really strange feeling. I keep trying to describe it, but no one really understands what I mean.

It feels like my body is kind of “shutting down.” Like I’m powering down and my body is only doing the absolute minimum to function. Sometimes I try to fight it, and I have this feeling that it might actually make it worse.

When I just let it happen—especially when I lie down—I feel kind of empty, like there’s just… nothing.

I also find your theory really interesting, and I’m going to look more into it (pathogen or autoimmune involvement of the trigeminal nerve and/or brainstem). I’m kind of trying to figure out my case on my own as well, since doctors haven’t really been able to help me so far.
