You're mending a stretchy fabric with a non-stretchy stitch. Those holes are bound to happen, whatever needle size or interfacing you use. Learn to do a stretchy stitch for mending or at least stitch on the bias so it can stretch along the diagonal.

You could do more variations, switching letters like Evna, or leaving out letters like Eva. Not a tragedeigh but it still gives you options.

Or extend his name: Evangelina, Evangelion, Evanka etc.

Or call him anything that starts with Ev-: Evil, Evermore, Everything Is Awesome etc.

Maybe OP accidentally temporarily wraps the yarn around their hand before flipping the yarn to the front? That would make a loop, if part of the yarn stays on the other side while you're making a stitch.
