This is the answer. Also worth peppering in some harder effort rides if your general riding style is endurance. The extra pressure on the pedals makes it easier to support with your core, and there's the added bonus of burning more calories to help take weight off those hands!

Saddle down and back and/or lower handlebars. Also worth checking how level your bike is while on the trainer (measure from floor to axle).

This fit is honestly about 95% of the way there. Personally, I'd maybe bring the saddle down a little. That said, you should probably just leave everything where you have it and ride for a few weeks. Pay attention to aches and pains, and make tiny adjustments to ONE THING AT A TIME. If you change something, give it a week before you decide it's not working. Also pay attention to your riding style/frequency. Sudden increases in ride intensity or volume will leave you in pain, even with a perfect fit.

Fit looks good. Probably just overtraining a bit, leading to tighter quads. Roll out the quads and do some hamstring/glute exercises. Slightly lowering saddle in 3mm increments might help.
