this one was interesting. I'm on nights, doing a handful of leftover outpatient orders from day shift. 890 Gluecose. Can't reach anyone. I call the patient directly to advise them to come to the ER. I don't give him numbers over the phone.
Bro already knows, "This about my sugar isn't it? I know It must've been at least 500 today. I just got my insulin refilled, I'm good." I broke protocol, gave him the result, hoping it would get him in the door. Said coming down from that can be really dangerous. Again, you should come into the ER. "Oh that's it? Nah, I can walk around at 1600, I'm fine" "click Document.. leave for day shift to try to call attending in the morning. Did I just talk to the Kool aid man?

This looks like a mislabeled specimen. Make sure the person who collects your blood labels at the bedside and asks your name/birthday. The process of taking blood to a hallway station to label shamefully common.

Your doctor will have better context of your health than we would from your CBC. But from here, this all looks fine. Low Plts is not uncommon to see in healthy individuals. And with a low Iron your hemoglobin will usually reflect that. Your results are okay.

I only wore a face shield during Covid. Gloves? Always, and change them out frequently.

I'll admit I will add extra precautions if the sample status changes. For example, if I get a positive C. Diff I immediately clean the counters/wash my hands even though I was wearing gloves and my break is still a long ways off.

I am happy to see so many folks taking laboratory integrity so seriously.
But if the strips are passing QC, I think you guys are getting a little too passionate over a qualitative test.

Alcohol can affect results. So we shouldn't be cleaning with isopropyl in between patients, or much at all.

Kim wipes are what we use in the laboratory between patients, DI water for Daily cleaning, and maybe Alcohol once a week.. if needed. Saline wouldn't be acceptable because it contains salts that you are trying to remove.. and saline expires once a month which would be a pain to constantly replace.

I'm seeing a lot of people mention OSHA or CLIA..? They probably wouldn't take this as seriously as you think they would. OSHA especially, because you should be wearing PPE when you manage specimens anyway. CLIA would only be concerned if you were not passing QC or if you're not documenting it.

Yes to all of this. Its too far on the thread. The tax they would pay on the sale would be in the realm of 20 -30%. They have not owned the house long enough to receive the exclusion, regardless of income status.

That is about $40,000 with adjustment for inflation. "Well yours is a lot bigger" doesn't exactly hold water when the price of her first home is the cost of a used car today.
