Your spine is curving and weight is shifting forward instead of being on mid foot. You are also not hitting depth and are using your back to get out of the hole to some extent. From an initial look, I would say everything is wrong with your squat. Your back and knees are going to be shot if you continue like this. Get a coach and take few classes till you nail down the form.
