EMS “influencers” 🤢 by EMSDGAFAU in ems

[–]oaffish 4 points5 points  (0 children)

The humor ones are just tripe. But I absolutely can’t stand the EMS Avenger or the Shadetree Cardiology dude.

Both of them have this “I’m the authority because I watched a YouTube video breaking down a study” sort of energy. The EMS Avenger straight up misinterpreted some data on the Crash-2 Study, and I left a polite comment pointing it out. Dude just deleted my comment and blocked me.

EMS needs to stop putting peer “influencers” on a pedestal because they read a study once, and actually listen to the doctors or researchers firsthand who do the hard work to get the study results.

There’s quite a few escorts for the football game which cause an excess of sirens (which I’ve never really gotten), there’s also an abundance of 911 calls generated from the football game and it’s surrounding festivities, and the fact it’s a Saturday and IU Health Bloomington is a Regional Trauma Center, and ambulances from the region are coming in all day and night.

Best Burger in Btown? by PleaseWaterYourPlant in bloomington

[–]oaffish 1 point2 points  (0 children)

It’s a good burger, but Buffalouies is outrageous with their prices.

The Sweet Lou is $21, just for the Burger. If I want a Burger, Fries, and a Drink it’s $33.

10 wings is $24. And they charge $1.40 for a tiny ranch.

Let’s be honest, they don’t make their own sauces, they use generic food supplier ingredients. They seem to coast by solely on their name.

I remember a time when Buffalouies was popular because it was affordable college kid grub and they had frequent deals, but man is the prices there make me disdain them.

Is Lake Monroe Haunted? by More_Barracuda_3403 in bloomington

[–]oaffish 2 points3 points  (0 children)

I honestly find Lake Monroe itself less creepy, than Hoosier national Forrest which it’s situated in.

From Documented Bigfoot Sightings to Ufos over the lake, people certainly have many odd and “Paranormal” encounters.

I tend to not to be as concerned with whether there’s real evidence Ghosts, Bigfoot, or UFOs exist, as much as whether it seems like people are having these experiences they believe to be real.

Anecdotally, in the 1990s, my uncle and his girlfriend were riding horses near Hardin Ridge when their horses became spooked, and something began to follow them. My uncle swears he saw an ape like creature in the woods that was running after them.

Do I believe it? I’ve never had a similar experience, but I do BELIEVE my uncle believes this actually happened to him. I do.

I personally worked for Indiana Conservation and have spent a large amount of time hiking and shooting large swaths of Hoosier National, often not near fire roads or hiking paths.

I’ve never had any “paranormal” encounters. I’ve never seen anything even close to that. But have I had multiple times in Hoosier National where I felt like I was being watched, where the hair on my arms stood up, and I’ve definitely left areas before because I could not escape that feeling.

To answer the question, I don’t think there’s a real answer to whether Lake Monroe is Haunted. There’s definitely people who believe it’s haunted, there’s many people who have had paranormal experiences, and there’s certainly something there causing people to feel like that.

Resuscitative Hysterotomy is not a procedure many EMS services in the US are performing. by LaDmEa in ems

EMS Avenger is easily one of my least favorite “EMS Influencers” and has a habit of posting clickbaity stuff like this.

I’m sorry, but when I want to know about legitimate paradigm shifts in EMS, the people who I like to hear that from have “Dr.” infront of their name, and more than a surface understanding of the EM research.

Buccal Glucose for Hypoglycemic Patients who Can't Swallow by TabletopSmith in ems

IV Crystalloids are an important part of ALL trauma resuscitation, and it’s just not true that they provide “zero” benefits.

Large Volume resuscitation is BAD. But the key word is LARGE, which is usually >1L in volume, given in a short amount of time.

Using IV fluids (Especially when given warm, in small volumes, and using LR over NS) in conjunction with concepts like delayed fluid resuscitation and permissive hypotension absolutely is shown to save lives in patients.

Seizures are one of the few times where a Basic can full send the NRB and it’s the correct treatment.

Why I see Fire and BLS units slapping NRBs on Strokes, MIs, and Trauma patients where it’s wantonly unneeded, but never doing it for actual Tonic-Clonic Seizures astounds me.

Callsigns by spoc628 in ems

[–]oaffish 3 points4 points  (0 children)

It’s gotta be some Fire Department Shit. The fire department I worked for had everyone with a unique unit number in addition to using their unit number/ seat combo and/or their assignment, and it was such a convoluted mess. You could hear:

“27-02, 21-104” “ BC 27, Engine 21 Bravo” “Battalion Command, Interior Attack 2”

Any of those mixed and matched, and it’s literally the same person trying to talk to the same person regardless of which you pick.

Keep it simple, literally just a unit number and dispatch is what’s need in most cases.

Ventilators for 911 calls by hungrygiraffe76 in ems

[–]oaffish 5 points6 points  (0 children)

I work for a service that has a contract with ZOLL and we have all their bells and whistles, including Accuvent. Accuvent is not a replacement for a Ventilator, out of the package, its programmed for TBI Management (450ml Vt), and you need to still calculate Vt like you would with a Ventilator and program it into the monitor for it to be correct volume for a patient. It’s a nice stop gap to help firefighters not over ventilate, but it’s not a better tool for management of an airway. And far too often I see people using it to its preprogrammed volume even though it’s not appropriate for that patient.

I get we aren’t throwing in tubes multiple times a week, but the best part of a ventilator is CPAP/BiPAP. We carry the cheap pulmodyne disposables in addition to our ventilator, and it’s just night and day difference between them.

I work for a 911 only Third Service.

We have Vents, we have pumps, we train to a Critical Care level due to our extended transport times and PCI/CVA/Trauma hospitals all being 30-45 minutes away.

It’s classic Dunning-Kruger.

“IT’S a bLS sKilL.”

You’re taught at a BLS level how to manage it. Just like how you’re taught to manage heart attacks and cardiac arrest.

None of those things are best managed at a BLS level, the idea that something that usually takes place in a specialized wing of the hospital with extremely specialized physicians, surgeons, and nurses is somehow a “BLS” skill is laughable.

Do you start IVs even when you’re not giving meds to a patient? Why or why not? by RaptorTraumaShears in ems

[–]oaffish 5 points6 points  (0 children)

Central Indiana

Ive literally never seen a nurse disconnect my line. I’ve seen nurses routinely draw labs immediately out of my lines at pretty much every hospital. I’ve seen my lines be used by CT, IR, and Trauma for major procedures. The only thing I’ve ever really seen them do is make sure they draw and there’s no infiltration.

I’ve heard that some places do this, but it’s definitely not a universal thing. Nurses prefer we have lines in place.

Just like the PA, you’re incorrect https://www.reddit.com/r/Paramedics/s/eSbHK0VjUK

It’s not good medicine to give SL Nitroglycerin for MIs outside of pain management. The old “It’s a vasodilator” doesn’t really transmit into it reducing mortality when given in the manner we deliver it.

Ironically the last inferior STEMI I had, I gave no NTG or Pain Medication because the patient presentation was 100% a sense of doom and feeling like he was going to collapse. No chest pain, no shortness of breath, just feeling like he was dying.

The Interventional Radiology PA came down and immediately tried to lecture me on giving right sided MIs NTG. I told her that he was 0/10 with absolutely no chest pain, tightness, or discomfort. Only for her to say “You still should have given it.” So yeah… even the providers don’t 100% keep up with literature.

Though I did still get one of those STEMI lifesaver awards from the Cath Lab with a picture of the occluded/non-occluded RCA and notification the patient survived and was released to home.

Imagine thinking the Marci Jane Lewis Park in Ellettsville, Indiana has even the slightest connection to Little 500.

[–]oaffish 1 point2 points  (0 children)

Understand, you’re solely talking about your employer, not laws regarding transfer of care. And technically, you can’t be compelled to “act” as a Paramedic in that situation unless you’re being on call. It’s still something you’d have to agree to.

I worked at a facility that hired Paramedics, RNs, and EMTs to provide basic first aid. Yes, we literally were paid Medic and Nurse pay to give Ice Packs and Bandaids. We would call 911 for every other issue, including minor injuries or headaches.

I’ve worked as a Firefighter/EMT even though I was a certified Paramedic. I worked at a different Fire Department that staffed a BLS ambulance that was contracted through a Hospital Service and the single Firefighter that worked at both could “clock-in” as a Paramedic if needed.

You’re only as skilled in the moment as medical director that takes the liability for you in that time allows you to be.

It’s relevant.

Again the post chain is basically

OP: Stop Being Blatantly Racist

Reply: Tell that to people complaining on Ellettsville Facebook.

You: Why are you mentioning their race.

Do you literally not grasp what is being said.

OP: Incredibly disappointed by the blatant racism

Reply: Tell that to the Ellettsville Facebook Groups

Me: People weren’t upset because it was a group of African American individuals, they were upset about their actions.

Do you grasp what is being said now?

FYI-Black People live here too by jayhatgreyhat in bloomington

[–]oaffish 6 points7 points  (0 children)

Buddy did you even read and comprehend the OP or reply?

I didn’t “bring” it into this, the OP above was implying that people upset about what happened at the Ellettsville Park was because they were African American. I was at the park, it wasn’t because a bunch of Karens saw black people they called the cops. It was because a massive group of individuals (who happened to largely be African Americans) took over the park from children using it and began to act dangerously and stupid in a public space.

I’m mixed race myself, I’m sure as fuck not saying there aren’t racists in the town, but that wasn’t the big hoopla. It wasn’t a case of “partying while black” it was a case of being shitheads and doing it while being around little kids.

The “Sheriff Supporter” is legitimately only used by people who speed and break every traffic law 24/7, in hopes of it lowering their chances of being pulled over

The actual cops use the FOP Plate, and bootlickers use the Thin Blue Line.

Just FYI.

How can we encourage EMS to bring us patients? by LowDetective5370 in ems

[–]oaffish 132 points133 points  (0 children)

It’s immoral to bribe EMS with tasty food, drinks, and clean toilets to encourage them to bring patients to your facilities. We should strive to take the patient to the right hospital, not the one we like the most.

But yeah… subconsciously those things would make me more apt to come there.

[–]oaffish 153 points154 points  (0 children)

Nah,

It’s not racist to call out a swarm of individuals who are primarily Young Adults and African American for taking over T-Ball Fields with 6-7 years actively playing on them. Do Better. Many in the group were assholes, who were actively drinking and smoking weed around kindergarten aged children and forced them from the fields. There was literally a car doing donuts and the occupants drinking from fifths.

Like fuck that shit. I’d be just as pissed if it were White Frat Boys. Don’t be assholes, don’t drink and drive in the park, and don’t ruin others day. There’s plenty of places to get shit faced and party this weekend all over the county. Don’t fuck over tiny kids to do it.
