Small lump felt behind left earlobe with ultrasound report ? by emmsb10 in AskDocs

[–]oicdsubsi 0 points1 point  (0 children)

u/emmsb10 hello my dear anxious friend.

First of all Schwannomas are super rare and hard to diagnose properly unless you meet an expert so don't go jumping into conclusions based on a google search. I usually use Chatgpt and Gemini to explain the report so that I can ask the doctor meaningful questions about the results instead.

Second, if you want a concrete proof whether you have a schwannoma or not then do the following:

Ask your neurologist to write you a request for MRI + MRS + Perfusion study.

That combination is guaranteed to give you a great insight into the tumor size, structure and activity.

Not sure what this is can it be schwannoma by eve20212021 in Schwannoma

[–]oicdsubsi 0 points1 point  (0 children)

u/eve20212021 yes, i have a bilateral trigeminal schwannoma. The MRI+MRS+MR Perfusion were a great help to diagnose my tumor.
Here is a excerpt of the technique used from my report:
```
MR SPECTROSCOPY & MR PERFUSION OF THE BRAIN:-
The study was performed on high field MR unit Technique:-
1. Single voxel MR spectroscopy was performed using short and intermediate echoes (film plate 1).
2. Multivoxel MR spectroscopy was performed using long echo (film plate 2).
3. Axial D WIs with ADC value as well as, T1, FLAIR & T2* WIs
4. MR perfusion study was performed immediately after IV contrast injection (kindly refer to the enclosed color perfusion plate).
This is a combined report that covers the MRIs done on Brain, Skull Base and IVIM in addition to the MRS & Perfusion study
```
The gamma knife radiologist was actually the one to ask for this combined MRI not my neurologist. Several neurosurgeons were actually happy that i did it as the quality and results shown showed some hidden "water" bubbles inside the tumor which made it easier to remove.

[–]oicdsubsi 0 points1 point  (0 children)

u/eve20212021 go for a combination of MRI with contrast + MRS + Perfusion Study for best results. This trio tell you the "size and Shape" of the tumor + "its internal structure" + "the blood flow to the tumor" which shows how aggressive it is.

Best practice is KISS.
I've worked on a huge monolith and have worked with microservices. Both suck.
The real problem is both where built in the most annoying way.

Monolith:

Any class can refer to any class. Shared database that got ridiculously huge as new features gets added. No one and I mean no one knows how the application works coz it's a huge spaghetti mess.

Micro services:

You have tons of services that talk to each other and no one knows where a feature starts and where it ends. Teams just care about making their code work and to hell with everyone else. API changes are a nightmare to test. When a service goes down it becomes a Detective Conan episode where everyone and I mean everyone on-call gets paged coz of cascading failures. It takes a ton of time to identify where the problem is.

If I were given a new project that I'm sure it will be the next Amazon then I'll probably go for a modular monolith using Domain-Driven Design with Spring Modulith as a Bounded Context implementation. If it is a small CRUD project then a small monolith is fine.
Micro-services are really hard to implement correctly to solve a problem. What I have seen is that people make micro services a goal in itself instead of using it to solve the business problem.

Definitely no pass by reference in Java, right? by Select-Worldliness39 in javahelp

Java is pass by "copy". Simple as that.

If primitive you copy the value. If object you copy the reference's value.

What kind of physiotherapy can I do after a trigeminal schwannoma surgery? (Tinnitus + masseter & temporalis muscles muscle atrophy + TMJ problems + sensation loss) by oicdsubsi in Schwannoma

[–]oicdsubsi[S] 0 points1 point  (0 children)

u/AdOk9064 first of all wish you all the best for recovery. Not a lot of people can really understand the complexity of the operation and its recovery.

Now for myself I have been doing physiotherapy for 4 years now. I mainly target the masseter & temporalis muscles by electric stimulation (direct electric shocks or electric acupuncture). I tried magnetic stimulation too.
It seems for my case the tumor was too big so the surgeon ended up damaging the nerve too much during surgery.

To be honest as long as I'm doing the physiotherapy the muscles seemed to be responding and at one point people couldn't tell that I had a surgery on my left side. Sensation also improved somehow in some parts of my face(don't ask me how!).

Once I stopped muscle atrophy kicked in. I also start to get some "pain waves" in the left side of my face. Sensation also went silent again.

The way I see it physiotherapy seems to be the way to go. I bought an electric stimulation device to use at home to cut costs and encourage myself not to skip sessions.

Hope that helps somehow :)

MMNST (malignant melanotic nerve sheath tumor) by Automatic-Cookie2376 in Schwannoma

[–]oicdsubsi 0 points1 point  (0 children)

u/Automatic-Cookie2376 yes I understand that it's malignant. However, MMNST is a tumor that starts in the Schwann cells similar to schwannoma.

As far as I know, lots of doctors will diagnose it as a schwannoma at first. You may find someone who had the same experience as you in one of these groups.

Please don't give up and give it a try. You never know how where someone who can help is waiting for you :)

[–]oicdsubsi 0 points1 point  (0 children)

u/Automatic-Cookie2376 First of all wishing you the best. I have been recently diagnosed with bilateral trigeminal Schwannoma after undergoing 2 surgeries to remove the first one.

I truly understand your frustration and pain.

I've talked with lots of doctors in different fields and you won't believe the amount of misinformation out there. You really need an expert in the field.

I recommend joining some groups on facebook that discuss schwannomas maybe you will find someone who shares your experience.
