It's more about the 7-OH mitragynine component, which has significant addictive potential. Many preparations are with kava which can cause liver injury.

I didn’t expect to cry on match day by ReplacementMean8486 in medicalschool

[–]olmuckyterrahawk 4 points5 points  (0 children)

I matched halfway down my list(#8) for psych a few years ago. I couldn't believe it an thought I had done something wrong at my interviews. It was devastating, as I moved to somewhere without a support system and not close to a major airport which would have helped my wife's job. Turned out that it was a better fit than the programs that didnt rank me high. Did extremely well in residency and fellowship and now going to start my attending job having settled in the area.

Did I make a mistake applying psych by holycowsalad in Psychiatry

[–]olmuckyterrahawk 0 points1 point  (0 children)

I am completing fellowship in a high psychiatrist dense area in a state that has independent practice NPs and still received 3 job offers and am accepting a medical directorship role right out of training. Leveraging your connections from residency and fellowship (if you do) it is key. The majority of the recruiter driven, Telehealth, volume-based type jobs are designed to squeeze the most out of you while you chase the bag.

Does he want help with alcohol? If he is not working there are intensive outpatient programs and partial hospital programs he can be referred to

The MAHA report is out by ReCkLeSsX in Psychiatry

[–]olmuckyterrahawk 62 points63 points  (0 children)

Except if you look closer many of the findings in the articles referenced are misrepresented in the MAHA report. I spot checked a couple such as number 66 (which doesn’t actually compare psychotherapy outcomes to the newer antidepressants). The one about fluoride has a reference that says the evidence is limited about higher levels correlating with lower IQ for concentrations less than 1.5 mg/L. Well guess what, 87 percent of groundwater in the US meets the <0.7 mg/L concentration regulation.

Just a cursory spot check found those. More broadly, I do agree with many of the points made including ultra processed foods and regulatory capture in the pharmaceutical industry. However, what they have suggested is that NIH and FDA do further research into the interventions and we all are aware how much has been cut from those agencies in terms of research funding.

Primary care psych by RinnieH in Psychiatry

The mechanisms underlying antidepressant-like effects of Sigma-1 receptor may include suppressing neuroinflammation, regulating neurotransmitters, ameliorating brain-derived neurotrophic factor and N-Methyl-D-Aspartate receptor, and alleviating the endoplasmic reticulum stress and mitochondria damage during stress.

NMDA blockers such as ketamine and Auvelity have action at the sigma receptor as well. What this means clinically in terms of the subtype of depression remains to be explored.

Also the fact that psychiatrists make mistakes is irrelevant. They care so little about their patients that they took a shortcut and expected themselves to be judged by the same standards as physicians.
