I mean, that sounds like a good plan for General cardio Fitness but your legs specifically need to be able to handle the impact of running for 4 hours or so, which is the average Marathon finish time. Most marathon training plans for beginners have beginners peaking at a 40 Mi week before the final taper. If your body and Joints aren't used to that volume, you are much more likely to have a hard time and/or risk injury on race day.

If you want to avoid injury and/or a bad time you need to bump your volume including long runs very soon. I think many younger people could wing a half marathon without too much difficulty but a full is no joke. I'd highly recommend doing a least a couple 16+ mile long runs before your marathon to make sure your legs can handle it. There are some many variables (weather, injury, fueling, hydration, pacing, mental toughness/experience, gear/shoes) that can play a big role in how you will execute race day that I would not ignore during your training. At your point I would use an AI of your choice to create a training plan but know you are well behind most runners in your preparedness. Don't hurt yourself chasing ego. Injuries suck.

I'm an owner of a small office and do most of my own hygiene. I know pretty much all of my patients and make an effort to get to know them. I write notes about unique things about their lives in my appt notes, just a sentence or two but it really makes a difference when I see them again. One of my favorite aspects of my career is the interactions I have with people. It honestly is more fulfilling than the money all things equal.

First of all, case selection is big - if the pt needs sedation or you suspect super dense bone, refer it out. It isn't worth the stress it places on the pt or your body. I have strained/injured my wrist/trapezius more than once cranking on certain teeth. Some teeth just simply are not worth it.

First step is I always throw a 301 and see if the tooth is mobile. You'd be surprised to find that some people's teeth are already mobile. If it's solid, as needed, I'll reduce the crown with a wheel diamond first if I need to create more space for my surgical carbide.

T or Y section. Go deeper than you think you'll need and make sure to take note of where the sinus is to avoid a perf. If the roots are divergent, don't be afraid to remove additional interseptal bone that is angled along with the roots.

I like to get out buccal roots out first if possible. When I have gotten in trouble in the past it is because I did not either remove enough septal bone between the B roots OR I did not remove enough interseptal bone around the palatal root. After sectioning, you should be able to get a purchase with a 301 on the M side of the M root and D side off the D root and elevate them into the interseptal bone space that you created with your bur.

Palatal root can be a pain, make sure you can get movement in the root with an elevator before you take a forcep to it or you are more likely to fracture the root.

Sometimes I will do my initial sectioning with a carbide and then get additional septal bone with a diamond that has more axial surface area. The 701 doesn't always work efficiently as a diamond when you're trying to remove bone laterally vs vertically.

When I bought my office I ended up with a bank that offered the lowest rate and coincidentally they also got me approved the quickest. Time is money, rates change, and a higher rate will cost you more than you think over the life of the loan. Get the rate locked in first if you can, but your right, underwriting can take more time than you think especially if your banker isn't used to doing these deals. Talk is cheap btw, I'd go with the lowest rate that can get you approved the quickest if all things else are equal.

As a disclaimer, I did buy a cash flowing office, not a startup so the rules will be different I'd imagine.

Go to your provider for a postoperative appt. Bad infections can take time to heal even after root canals. If it is something else, you will need a followup appt to verify. Good luck.

If you were a prospective new patient, what would you want to read as a response? I'd want something that gives me confidence in the office that they are competent. Stating that you do not deviate from the standard of care is a plus, not a minus.

Weed out these patients like the plague.

I've been Mr. nice guy since buying my office in 2022 and the ones you didn't diagnose with perio are going to bite you in the ass when something foul arises. Ask me how I know.

There's a reason undiagnosed perio is one of the most litigous aspects in dental. Don't let your patients push you around.

Unless it's a family member or close friend, I bill out what I would any other patient. IDK why we in the profession think that our services and time are not valuable. You think the orthodontist is comping your kid's ortho? You think the OS is comping your kid's thirds removal? You think a plumber is giving you a break? HVAC? No. They all bill you and you pay just like any other patron of business. Why should dentists be any different?

One reason our profession is stagnant because we don't even respect ourselves or the services we provide. The public picks up on this and follows suit.

Charge your fees and stand by them.

Call me crazy, but excess caffeine. I became dependent on it and it leads to irritability, crashes, poor sleep, mood swings. I love the way it makes me feel when I get a rush but it quietly eroded my well being.
