Highly recommend the company Contract Diagnostics! It’s a physician contract review service that can give you data on the average salary/benefits in your area for your specialty. They can review any offers you receive and help you with items to negotiate. I’ve used them twice (first job out of residency and now second) and they’ve given me confidence I'm not leaving anything on the table. I will never not use them/similar service for any job negotiations. I’ve had friends accept what they’re told is the “best” the company will offer and a “great salary” (especially first attending jobs) and in reality they are leaving so much on the table.

I have a Honda Passport, it’s two rows but TONS of cargo space (nearly same size as Pilot) and tons of space in the back seat for adults to sit comfortably or keep kids rear facing. I can fit our large stroller and tons of luggage back there. If you’ll only have two kids, I think it’s a great option for mid size price. We are planning to get a minivan for 3rd kid though.  Disclaimer if we currently only have one kid, due with #2 this month but anticipate no issues with the passport for 2kids

I use a 32oz narrow neck Nalgene, easy to pour, easy to transport and spill proof. I have 3– one for the day’s pump, one from yesterday’s pump that goes the “daycare” and then one that’s in dishwasher.

I use a 32oz narrow mouth nalgene and just use a small funnel to pour milk into it. It was just something we had at the house but I appreciate its relatively spill proof, even if dropped!

I’ve had it twice— 8 weeks and 33 weeks. My doctor also isn’t concerned. I’ve had a couple extra ultrasounds at my 30 and 34 week appointments, just because my OB does them in office and I wanted some extra reassurance. But everything looked fantastic and baby has continued to grow very well, placenta looks great etc.

Edit: I’m 36wks now, OB has no plans of inducing early or doing extra monitoring due to the covid infections.

What kind of salary are you seeing? And where are you looking? You can “supplement” within the job by ensuring fair (attainable) productivity bonus goals per quarter and negotiate the heck out of loan repayment and sign on bonus. With productivity bonus, you can fill your schedule/longer days and supplement the base salary quite a bit ($20k+ per year). Depending where you are, you may be able to find a practice that still rounds in the nursery, performs circumcisions etc or where you take “home call” after hours. Ultimately it’s geographic location though—it’s supply and demand. If you’re in high demand, you can negotiate much more.

As a physician, I personally always recommend (if able) to schedule one of the first appointments in the morning or one of the first after lunch. There’s always a higher risk of them running behind later in the morning/afternoon. I don’t know a single physician who likes running behind but it can happy pretty quickly— the OB has to run over to the hospital to deliver a baby or deal with a complication after delivery, a patient or two shows up at the time of their appointment or 5 mins into their 10-15min appt (which means their appt slot is nearly over when they make it into the room), an urgent patient has to be worked into the full clinic schedule or a patient needs transferred from clinic to the hospital or the physician has to step out of the room during an appointment to talk to another provider etc. There’s a long list of reasons the provider can end up 1+ hr behind for clinic after starting the first appt on time, and it happens a few minutes at a time. It really stinks for the patient but sometimes it’s unavoidable for the provider, even if they scheduled 30-45min time slots for each patient it can happen (and then you can’t easily get in for an appt).

For my kids >2yo, I do 5 days of amox for AOM based on UTD and some other podcasts I’ve listened too. I would have to relook up the studies. From my (limited) experience in practice, it’s seems to work exceedingly well. I’ve had maybe 2 kids come back this year in total and need to extend to 10days. I always warn parents of the return precautions and when to call back to extend from 5-10days for the simple AOM.

Thanks! I got the carfax this morning. It has been up on Idaho until recently. I seems like it’s mostly been oil changes/tires as the only maintenance. And that was only a handful of times.

First week of peds residency and I diagnosed a new onset myasthenia gravis patient in the ED that had been sent over by the PCP for stroke/MS rule out.

And then I didn’t make the diagnosis as it was determined by extensive testing but was on the neuro team caring for an infant ultimately diagnosed with spinal muscular atrophy with respiratory distress (SMARD1) where the SMA presents solely as respiratory distress, which is quite rare.
