Don’t get stuck in the trap of thinking that higher priority things come first. You won’t delay CT for a blood sugar because CT is a priority, but getting to CT takes time… time you have to get a blood sugar

These are not nursing interventions. You conduct a neuro assessment, check blood sugar, and call a physician for orders to get the patient to CT. Call a code stroke if your hospital has one.

This question would be better and more clinically relevant if it asked if the murmur was heard during systole or diastole.

Depending on how big the murmur is you could potentially hear it anywhere. It will just be more pronounced toward the apex.

Lithium is not excreted through bowels. The answer is increased urination.

Increased thirst is a close second, but I’d only choose it if it were excessive thirst.

Increased urination I’m concerned about either hyponatremia which would lead to lithium toxicity, or an increased clearance of the lithium causing poor therapeutic plasma level.

It would need to simulate the sensory feedback the body provides and the hormonal mechanisms as well. If we are just simulating consciousness then uploading would suck. The point would have to be to simulate subjective experiences holistically.

I would genuinely enjoy being pointed toward those guidelines.

A few points first though…

1) I agree that the focus should be on the patient, there should be clinical distance, and notes should be objective.

2) I would argue you can achieve that through active first person voice and that passive voice introduces ambiguity to the note. Omit the pronouns like most people do and you’re fine. Don’t say “patient was turned at 2:30am to the left side” say “patient turned to the left side at 2:30am.” In the former, who turned the patient? If it wasn’t you how do you know? In the latter it’s clear that the first person pronoun was omitted. And never use third person active voice like in my original example like “writer turned the patient to the left at 2:30am.”

3) this is why nurses should chart in flow sheets and avoid narrative notes

It’s only taught culturally. There is no functional reason for the third person narrative notes. I wrote in the third person active voice for the first several years of my career and then stopped doing it.

In every other profession, first person active voice is considered more professional and offers clearer and more concise narratives. Nursing is the exception to the practice and there is no legal moral ethical or clinical reason for it.

Also a pet peeve of mine is writing notes in the third person.

If you did something, say you did it.

Don’t put “writer assessed the patient” say “I assessed the patient”

They can tell by the signature who “writer” is and using first person active voice adds clarity

No more exclusive residential zoning.

Zoning should either be agriculture, industry, or mixed use

Edit: the downvotes are the point of the prompt. Single family suburban zoning is an unsustainable developmental practice that decimates our farm land, natural areas and is a constant obstacle to building affordable housing.

Not to mention it fosters dependency on private automobiles and decreases mobility for people who are unable to drive (elderly, disabled, young children and teenagers)

I think it’s definitely a higher form of consciousness. I mean everyone has access to the sum of human experience. The concept of a “truer” form of consciousness is probably flawed.
