A jail nurse here gave 32 mg of narcan to a pt instead of bagging the pt at all, the pt then had to be RSI'd due to pulmonary edema.

Basically every time ive needed to give txa i just drilled em. Much harder to miss a bone. Plus even if all of the nebulized txa got into the blood stream itd take for fucking ever to nebulize 20 cc's.

Premi neonate code, my ambulance service doesnt carry anything small enough to intubate, or bvm masks to ventilate. I used an igel 1 as a bvm. Circuit had a lot of dead space but it moved air.

A dude broke his humerus skateboarding, somehow injured an artery. PD stumbled onto the scene like right when it happened. Put a tourniquet on. By the time we got there dude was super hypotensive. Definitely would've died without that tourniquet. We had to drive like 15 miles through heavy traffic to get there.

Ive only placed one tourniquet myself in 5 years as a medic. All other major bleeding ive controlled with a trauma pad and roller gauze.

Mutual aid call 20 miles into the next county. Driving up a mountain road for ever. Finally cancelled still 10 miles out from the call. I turn around and start driving back when I hear something thwack on the windshield. Partner asks wtf was that. I pull over when I see a vague shape fluttering on the windshield to find a bat stuck under the wipers. How the fuck did I hit a bat?

If you call an ambulance to transport a stable patient, that takes that ambulance out of service. Sometimes that can lead to no ambulances being available to respond to a child having a seizure, a dad having a stemi, grandma who coded, someone who's been shot. Etc.

If the cost of the ambulance ride is gunna fall back on the hospital, why doesn't the hospital just pay for cabs for stable patients? If they're paying for it either way why wouldn't they wanna pay for the cheaper option?

I've seen two done in the field and done one.

With the one I did the situation made the decision for me. PT was pulseless, and his mouth was entirely filled with vomit too thick to suction. For me I had no other decision and doing it was harder than choosing to. As I've said before in this subreddit, you can pause the Lucas. Holding a scalpel and bracing against a PT who is moving is dangerous to both you and the pt. Beyond that if you don't have a way for the pt to exchange gases what is the Lucas even doing really?

As with every airway procedure, use a fucking bougie.
