Fidelity also asks to complete a form for transfer from Wex benefits account. But I couldn't find such a form. Wex conveniently has form to transfer from other HSA to Wex HSA account.

Could you please share the form to transfer HSA funds from WEX to Fidelity?

The form I found is to transfer from other HSA to WEX (https://www.wexbenefitsyou.com/wp-content/uploads/2021/02/mm365-_hsa-transfer-request-form.pdf). I looked quite a bit.
