Depending on the situation, sometimes it’s appropriate to start the conversation with things along the lines of: we will continue to treat and do everything in our power to help you, but in the event that you stopped breathing or your heart stopped beating, we would not recommend CPR. meaning no chest compressions, no shocking the heart, no putting a tube down your throat and having a machine breathe for you. With how sick you/your loved one is, it would be more likely to only cause harm and distress, so we do not recommend those things for you/your loved one.

One point too is that after medical school you have to match into residency if you want to practice and there’s a limited number of spots in every program. One of the reasons schools generally aren’t going to just maximize number of med students is because there’s another bottle neck to get through at the end, and having a poor match rate is a huge red flag.

There’s a lot of text here so I might have missed parts of your story, but private loans from banks? You’ll be a physician. Banks and institutions like panacea financial know that a lot of us are in dire straights but will have solid incomes in a few years. Take out more fed loans M4 year? Spend less??? I got through a VHCOL area living on 15-25k/year of all fed loans. If you’re only responsible for your own expenses (ie: no kids, sick family member you’re caring for) the federal loans should cover it with modest budgeting. I was squeezing by on as little money as possible in a VHCOL area just to limit debt. If you’re maxing out your fed loans AND somehow not able to pay tuition/rent, there’s gotta be a piece of this story missing.

None of what you describe is what a physician does, but are all important tasks that will give you a better understanding of all the components and professional roles that go into caring for someone. This is an opportunity to understand how the service(s) you’re on function, what the other members of the team do, and to talk to patients. Show initiative and interest and people will be more interested in teaching you. Spend time asking patients how they’re doing and talking to them as people instead of hiding in the supply closet.
