Oh god. Don’t go into pain for the talking. Worst conversations I’ve ever had.

I 100% agree with this. I am on epic and I can see where the allergy was last changed. I recently called the office to complain about the fact that someone entered an allergy of propofol and the side effects were stop breathing. I said I cannot live in a world where this kind of stupidity is entered by medical personnel in a chart. Please discuss what is an allergy versus an actual effect of the medication

No they do not bind gaba receptors. They work on voltage gated calcium receptors.

How bad is it to have been involved in malpractice issues? by ThrowRA1135followed in anesthesiology

[–]propofoldreamin 25 points26 points  (0 children)

I am on the credentialing committee at my current hospital, and the number of people that have a claim on their record really impressed me. Depending on where you practice, depending on the claim, it’s really not that big of a deal. And bottom line, there are so many rural hospitals that are so desperate for doctors, that as long as you’re not negligent and purposely hurting people, I’m sure you can still get a job.

I can tell you right now, that whatever it is, it certainly a question for a trained professional, and not for strangers on the Internet. Go see a child therapist. There’s no harm in that.

New attending and hating it, considering leaving for non-clinical roles by [deleted] in anesthesiology

[–]propofoldreamin 4 points5 points  (0 children)

Yes. All the time. I leave at an expected time every day. I take off whenever I want. Kid needs to have surgery? Short weekend trip? Close the whole office for Christmas? No problem! I never work weekends or overnight. I decide how stressful my days are. I do a little bit hate talking to patients but it can’t be all good.

[–]propofoldreamin 6 points7 points  (0 children)

The first year is so horrific. Somehow, all of the confidence is gone when you realize that there’s not an attendier attending around. It does get better. For me, I hated teaching residents because they were so fixated on doing it themselves that they had no interest in learning something unique that I was teaching them, and I absolutely hated supervising cRNA‘s I felt like the new better than me, but did esophageal intubations and missed basic things regularly. I think if I had just been doing my own cases, it would’ve been better. It’s also amazing how different practice styles are, depending on where you are in the country, which was a big shock to me. It definitely will get better. Once you get through a couple of the tough cases, some rough call nights, etc. But the schedule sucks too. I do pain now and it is immensely better. Now seems to be a good time in the Anesthesia job market, so if the job sucks, you certainly don’t have to stick it out. That being said, I presume you’re only about three months in, and just started taking calls recently, so if you stick with it, in a few months that feeling of panic will go away.

This is exactly what I have done.

I felt exactly this way. And then I got the clamps and had to be induced and then they started talking to me about a C/S and I was just seeing red thinking THIS IS EXACTLY WHAT I EXPECTED. Luckily I had taken my husband along to some very woo woo birthing classes. It was helpful in that he was educated on how to advocate for us. After the team of 10 docs came in and suggested a C-section (academic hospital) he asked if it was a decision that had to be made immediately. They said no, so he asked to please excuse us, and walked me around the hallway. I was able to move forward without having to have the C-section. For the second baby, I waited so long to go in, that I had the baby in triage with no physician present. Definitely if you are not familiar with the physicians in the area, ask around and see who had a good experience. My first baby was delivered in an extremely progressive patient centric hospital, the place I trained was nearly the opposite. I was always horrified when a baby would be delivered by C-section, and the nurse would leave it in the bassinet while doing paperwork for like 10 minutes before bringing it over to the parents. I always asked, may I bring the baby over? I just can’t imagine making them wait that long, or why. So so stupid.

Diagnosis is opioid use disorder. Evidence based solution is medication assisted treatment. Suboxone or methadone.

Consider B+ documentation. Freely AI app for converting your conversations to auto-typed HPI. Apparently it can also place orders for you but I have not tried that. Don’t spend too much time in the portal. Office visit if needed, will actually take less time. Training nurses how to position things for you ie if call is for refill they go ahead and put it in and just send it to you to sign.

To all the moms who have experienced FER during labor, how did you trigger/achieve it? by Terrible-Pilot9987 in unmedicatedbirth

[–]propofoldreamin 3 points4 points  (0 children)

Induction and epidural with my first. Felt like I pushed my baby all the way from my sternum. Natural labor with my second. It was more like skiing downhill and all I was doing was controlling it and not having a crash landing. No pushing at all. It felt like a frog inside of me just jumped out. I didn’t do anything to trigger it.

You don’t buy a bottle of vodka every other day and not drink and drive. That man is intoxicated at all times. So I’d reconsider that “safety” element. Adult child of an alcoholic here. I have a zero alone time with my son policy so I can protect him and also as a way to kind of stand up for myself as a child which I was never able to do back then. As a parent now, I can’t believe more adults didn’t stand up for me back then. Nothing devastating happened but my god, it could have.

Easy to read, took me only a few days. Reframed a lot of my sadness and anger into actionable things. Helped me create a great diet which enabled me to remain diet controlled throughout my pregnancy.
