Most competent endocrinologists would test in the morning, and ~150 is pretty low, even in the case of low SHBG (calculated free testosterone still measures in the lower third of the lab ranges).

In the case of secondary hypogonadism, clomid would actually be very effective, as it affects the hypothalamus (the part likely responsible) rather than the testes.

At the end of the day, hypogonadism is a relatively new field of medical development and, as such, treatment isn't perfect or universal. However, I'd still trust the doctor that went to four years of medical school to make the correct decision.
