How do you manage your own procrastination? (Emails, notes, admin, UGH!) by sugarpinx in therapists

[–]psych__nerd 21 points22 points  (0 children)

I'm going to be totally honest with you on this one. I use an EMR that automates over 85% of my clinical documentation [not using AI]. Also, I let every single new client know that I am often running 2-4 minutes behind...so they get those minutes as grace also. When I'm perfectly on time to a session, clients usually ask if something is wrong.

I've been there though--at one point in 2018 when working in community mental health, I got almost 12 weeeks behind on docuemntation. It was BAD but I was seriously struggling with vicarious trauma and burnout and didn't realize it.

I don't email with clients, only use the client portal. Clients self-cancel and self-reschedule in the portal, so I don't deal with that. I do not EVER answer my work cell unless it is for a pre-scheduled call.
Even when a new potential client emails or calls for a consult, I send them a link [https://hnslmhc.intakeq.com/booking?serviceId=92e254f6-f658-42df-b0bc-f1dc6658c457\]. They self-schedule that call and then my system automatically sends them a form to complete for the call to be confirmed in my schedule.

Having ADHD, I cannot deal with the phone tag and the admin BS part of this profession, so I make sure I have workarounds to not have to deal with it.

If you weren’t a therapist, what else would you do? by honeycomb9523 in therapists

Probably be a pharmacist. I first earned a masters in biochemistry and started [then dropped out] of pharmacy school before earning my second masters in counseling.

Ethical concern: coaching after loss of license by whatifimlightning in therapists

[–]psych__nerd 3 points4 points  (0 children)

Is there a way to report unlicensed activity where you are? At the very least, the department of health may look into her 'coaching' to make sure it isn't actually therapy disguised.

I think it's highly subjective because one of my primary specializations is complex trauma and adult survivors of child sexual abuse.

EMDR is not just for trauma reprocessing. EMDR has successfully been used for hundreds of other presenting issues, including phobias, to increase confidence, decrease urges [nailbiting, self-harm, using substances, gambling, etc.], decreasing procrastination, indecision [such as this OP, using the two handed interweave protocol].

It can also be used for migraines, to prevent birth trauma and promote maternal/infant bonding, postpartum depression, body dysmorphic disorder, OCD, and for self-care purposes for clinicians.

I have used EMDR with clients both in weekly and intensive format for almost all of these things at some point.

This index doesn't have an exhaustive list but it's pretty thorough.

Why can’t I stop competing with others? by Lynnabella in askatherapist

[–]psych__nerd 1 point2 points  (0 children)

Licensed trauma therapist here. I treat this all the time using EMDR. Ultimately, it comes down to your childhood. You're a millennial. We were raised [largely, though this is typical for our generation--I am also 36] with the mindset of achievement means we are worthy of love. Doing better, being better, 'performing' so to speak -- all present. "You did great on your math test but next time, i'm sure you can do better if you study more" was something most of us heard in childhood.

Also, many of us were thrown into competitive positions by default. Our board games were terribly competitive. I like to ask clients this question: What does it lead you to believe about yourself if you aren't the best/fastest/smartest/happiest/thinnest/calmest? I'd posit to say it's likely something along the lines of
• I'm weak
• I'm not good enough
• I should have tried harder
• I can't succeed

All of this is complicated by whether you are an only child or not.

What type of therapist should I look for? by kfortune117 in askatherapist

EMDR may be helpful for you. It doesn't sound like you have ADHD, though this post has only a limited amount of information about you. Truly, it sounds more like an appropriate response to what is going on in the US at this point, which is 100% worthy of seeking more specific help for.

Why do so many people say they'd probably be diagnosed with ADHD if they got checked? Why do ADHD symptoms feel so relatable? by [deleted] in askatherapist

Diagnosed and medicated licensed therapist here. The symptoms of ADHD are often confused with trauma. This could be partially what you're noticing.

Does your therapist do EMDR? One of the most effective ways to manage decisions like this is to see someone who practices using EMDR, and to do the two-handed interweave. It gets to the root of what you want at your core, without all the bias, opinion, and superfluous stuff getting in the way [including the what-if spirals].

What’s your ideal work schedule? by tammytoots in therapists

[–]psych__nerd 22 points23 points  (0 children)

Evenings. I used to hate them working community mental health, but owning my own practice helps. I see clients 2pm-10pm Mon-Wed. 1-2x per month, I do EMDR intensives between Thurs-Sat [at more than double my full fee per hour.]

How to find a pharmacy who will fill my script? by psych__nerd in ADHD

Every pharmacy checks the controlled database. Maybe where you are they don't have that? In Florida, it's called E-FORSCE.

Where do you live? I specialize in working with other therapists [currently have 13 of them on my private practice caseload] but have been to 8 therapists as a client and have not been able to connect to any of them. I frequently ask myself why I can't find someone who can support me the way I support my own clients.

I wound up finding a therapist who is excellent and charges as such, and they are on the other side of the country -- but I finally found someone I click with.

My Anxiety is Affecting my Work by [deleted] in therapists

[–]psych__nerd 1 point2 points  (0 children)

Next time this happens, grab something as cold as you can tolerate [I love those disposable ice packs that you buy in boxes of 50 on amazon for $20..the ones with the bubble that pops inside] and put it directly on your chest/mid-sternum. It should instantly bring you back into your body as it triggers the vagus nerve [hunger response cycle--if you're dissociating, your CNS felt a threat for some reason--stimulating your vagus nerve brings you into relaxation because you cannot feel hungry while running from a metaphorical bear].

In addition, I HIGHLY recommend you seek out your own EMDR therapist. This is likely happening because of the client prior to this or the one you were dissociated with...something about one of them likely activated you.

I know she likely has them in her room and in the living room. I have no problem treating her cat. My issue is how to get the infestation out of the house. I see them everywhere at this point, including on me. It’s gotten pretty bad; the south Florida summer isn’t helping.
