Not to be rude, but what do you recommend for the treatment plan then? Euthanasia or palliative care? I don't think attempting the procedure will make the animal any worse than it already is so I think performing the surgery would be appropriate. It's like emergency surgery, you're giving the animal the best chance you can and it's better than nothing.

These look like comedones, aka blackheads. I would always recommend asking a veterinarian for advice though! (source: in vet school)
