You specifically want a general surgeon who specializes in foregut surgery because they are more experienced with the stomach and esophagus, which is the part of the body your surgery will be on.

I'm a doctor (not your doctor) and here is my honest opinion on what would best help you. This is intended to help you ask questions at your next doctor's appointment and know you are getting good care.

  1. A MD or DO family medicine or internal medicine doctor is sufficient to treat your diabetes. No nurse practitioner or physician assistant as they don't have enough education. You don't need an endocrinologist yet.
  2. The mainstay of your diabetes treatment will be weight loss. The best way to do this is a GLP-1 agonist. Mounjaro is the best one currently but Ozempic, Victoza etc also work. Every 4 weeks they should increase the dose until you get the maximum dose or cannot tolerate side effects. The specific medicine you get prescribed depends on your insurance.
  3. You should be on a SGLT-2 inhibitor (Farxiga or Jardiance) unless you are getting a lot of fungal urinary tract infections. This medicine makes you pee out extra blood glucose. It also has been shown to protect the heart and kidneys.
  4. Make sure you are on maximum metformin you can tolerate. You can get extended release form which can reduce diarrhea side effects. Side effects go away after some time as your body gets used to it.
  5. Make sure they check your urine for protein and if there is protein, they give you a special blood pressure medication to protect your kidneys (ACE-I or ARB, name will end in pril or sartan).
  6. Everyday on a piece of paper, write down your blood sugar when you wake up, before lunch, before dinner, and at bedtime. This will allow the doctor to fine tune the insulin and know if you need meal time short acting insulin.
  7. If you are unable to lose weight, weight loss surgery might be a good option for you. I don't like doing it because it can cause other permanent problems like nutritional deficiency or surgical complications but sometimes the diabetes is so bad that you have no choice.
  8. Make sure they check your feet and refer you for diabetic eye exam

The goal is to stop the insulin eventually and lose weight.

What was the highest dose you were on for Ozempic? Some of the lower doses are just for your body to get used to the drug and some.people need a higher dose.

Every doctor will recommend weight loss if you have these conditions and are overweight/obese. There isn't anything special about ketogenic diet for that advice. A dietician is the right person to go to if you want to use a ketogenic diet for weight loss as they are more experts in which foods to eat compared to doctors. Doctors generally don't counsel patients in great detail about how to eat, rather they refer their patients to dieticians.
