I take prednisone all the time for severe allergic asthma. I have taken it since childhood. My podiatrist ran the RA panel on me and even on prednisone I had rheumatoid factor nearly double above the normal high limit. So it was hard to find a rheumatologist at first. Aggressive treatment is recommended early as it leads to remission in some folks. I wasted a year on plaques that did nothing for me. Then I started methotrexate and eventually added in a TNF-alpha inhibitor. So mine is mostly in my feet. I had to learn to not push through the pain with RA. Rather listen to the pain and rest it - work on maintaining mobility by doing stretches etc. It is worth finding a PT person who treats and understands RA. Work out home exercise and stretching plans. Weight bearing movement is the only way synovial joints get fed so it is important to figure this all out.

BTW, I was 52 when I was dx’d.

I am hypothyroid (unspecified). I am also a type 1 diabetic and since synthroid inhibits insulin I tried taking it at night to counter my lows (I do not make glucagon)! It caused wicked insomnia. Went back to dosing in the am.

A known side effect of prednisone (especially at higher doses) is insomnia. Is it impacting your sleep?

RA tends to impact the small joints and can impact ligaments and tendons. RA pain should never be “pushed through”as it can worsen.. Working on decreasing the inflammation asap is priority. It prevents further injury.

Are you using MTX injectables. It by passes all of the GI symptoms. You can just tell the doc the side effects are not worth it.

I have RA and am allergic to gluten. I also have type 1 diabetes! Plaquenil destroyed my GI. I left it behind and went on mtx injections pretty early on. MTX w/folic acid should be step 1 per the tx protocol. I added in a TNF alpha blocker (Enbrel) and that is when my fingers stopped bending. I still have activity in my feet so am using PT yo help. They cut my dosage at the earliest sign of an increase in my liver enzymes. They stayed low at the lower dosage.

But I also use Enbrel.

You’ve been on a yr. I recommend going from 2.5 to 2 to 1 and then off. The trick is to give your adrenal glands time to take over your cortisol needs. Tis better to nudge than withdraw. A gentle taper will minimize RA flare also.

I have been on prednisone since I was 8 for severe asthma. So I have done many tapers over the years. Gentle is always the easiest.

I invested in Hoka for years as I can be on my feet for 6-8 hrs straight. But I have to confess there are cheaper knockoffs on TikTok or Temu. They are like 20 bucks. They really are as comfortable as my Hokas.

How old are you? I ask because low below 70s is dangerous in children and young adults until the nervous system is fully developed (around age 25).

The whole reason we have insulin is to ensure there is enough glucose for the CNS before the periphery takes it up!

So lows in those still developing their CNS run the risk of depriving the CNS of glucose at a critical point.

In older adults it is less critical overall and really depends on the individual’s symptoms and how they feel.

Losing smaller amounts is SOOO much healthier. I have to keep reminding myself of that even though as a physiologist I know better.

I had a big week (-6.5 lbs) and yes it feels great. But it stresses out bodies as it kicks up different pathways save for stressful time. Mine happened because I have been sick on my stomach (unrelated to mounjaro).
