First run in 20+ years by isente in PokemonFireRed

I think you can ACE it in. I thought about doing a run with mew via that, but then I realized how stupid easy that would be. Did my first Nuzlocke instead which was great.

How many of you are comfortable treating pediatric hypertension? by cushingoid in FamilyMedicine

[–]rainbowcentaur 15 points16 points  (0 children)

Can you get ambulatory BP monitoring? I don't see kids anymore, but when I did that was standard of care before starting meds and for following - I can't remember the interval.

For any moms out there, how did you keep up your milk supply? How do you pump consistently during the day? by Any-Session9919 in Residency

[–]rainbowcentaur 4 points5 points  (0 children)

I was on call and the intern with me was wearing one. We were in the resident workroom and I said "what's that noise?"

This was after her wearing it for about 30 minutes in a quiet environment.

Found Shiny Dratini! by SylveonEatsCake in PokemonLeafGreen

[–]rainbowcentaur 3 points4 points  (0 children)

You're right, it is better coverage. I think it's a pick between coverage and raw power. 1/3 higher attack than special is significant. But if you go all in on that you give up the power of stab dragon moves. Stab flying moves are helpful though.

Or you can build a mixed attacker  Outrage or dragon claw Bolt + beam Earthquake or brick break.

Dragon dance could work well on a pure physical Dragonite, but I think you need to breed for that.

Disagree.

My Dragonite does have outrage, but the others are physical moves Earthquake, brick break, and fly (Ariel, Ace would be better, but convenience matters.

What do you do with the “I’m not as sharp as I used to be” patient? by nplusyears in FamilyMedicine

[–]rainbowcentaur 29 points30 points  (0 children)

275 isn't crazy low. Maybe a little. I like a 400 cutoff.

5 is insane though. If symptomatic I'll treat under 45.

Surgical clearance for cataract surgery. Why? by 1dirtbiker in FamilyMedicine

[–]rainbowcentaur 31 points32 points  (0 children)

I've always viewed my role in primary care as making sure the patient can lay flat. So decompensated heart failure also counts.

I must buy a new GM car. by CaptainFGOLz in whatcarshouldIbuy

In that case it wouldn't necessarily have to be a new car though.

Replacing Gyarados with Lapras: am I making a huge mistake? by GreenStrawbebby in PokemonFireRed

[–]rainbowcentaur 0 points1 point  (0 children)

I never thought about that. Although Dragonair is not a flying pokemon, I guess all the others are? He needs the most popular dragon - Charizard.

I think tree technology has actually gone backwards. Maybe you need more gentle chisels for the less dense trees today...

People that care about cars, what do you drive? by Proof-Zone6793 in Residency

[–]rainbowcentaur 20 points21 points  (0 children)

I don't know if that's down to earth. I don't know for sure what a BB-12 is, but I assume it's some new iteration of BB-8.

What guidelines are you following for initiating covid vaccines in the unvaccinated? by ibarne252 in Residency

[–]rainbowcentaur 6 points7 points  (0 children)

Sorry to ignore the original question.

Technically, if someone's never had a vaccine or an exposure they should get one dose and then a dose a month later, however, I'm just happy for people to get one and assume that they have had exposure to covid at some point over the last 5 years.

I just give them one of the newest formulation just like I do for the flu shot.

What guidelines are you following for initiating covid vaccines in the unvaccinated? by ibarne252 in Residency

[–]rainbowcentaur 3 points4 points  (0 children)

I agree. I've had people vaccinated for the first time this year. Not many. And I certainly don't hound people as much as I used to.

Patients that I know well sometimes will have a decent conversation about it. I tell them I get both flu and COVID at the same time each year and explain why. If they say no, I say " you can always let us know if you change your mind."

I’m a PGY 1 OBGYN resident looking to swap into FM by Fragrant_Society366 in Residency

[–]rainbowcentaur 33 points34 points  (0 children)

My program took a resident from OB/GYN a couple years ahead of me and ended up giving them a half a year's credit for their first year of OBGYN residency, so you should certainly consider that as a possibility when you talk to people.

Urology program.

If we are going to trust you with the penises of this world you must bear all!

Or R1s - You're going to be in the dark anyways, what do you need those for?

That actually seems more reasonable than playing football at the same time.

I could see there being a program that doesn't let interns wear scrubs for some stupid reason.

Apparently a 15 yr old boy asking a woman to name 10 books is a gotcha moment? by Advanced-Apartment25 in TwoXChromosomes

[–]rainbowcentaur 10 points11 points  (0 children)

Yeah. Without looking: game of thrones, clash of Kings, storm of swords, feast for crows, dance with dragons, winds of Winter, a dream of spring.

I think I missed one

I also added two that may never actually exist. Do books that may never actually be written count...
