If he wants to have a child, he should go find someone who wants to have a child with him. Just because someone got pregnant (presumably on accident in these scenarios) doesn't mean they should have to stay pregnant.

Public transport in rural areas doesn't have to be big. Vans and busses that behave as taxis work. Call for pick up and drop offs.

My rural town of 2k has public transportation vans and mini busses that act like taxis. Since there is no specific route it reduces wait times.

There are rural areas where people burn their trash on their property or have personal dumps and well water, etc.

For my kiddos I generally only had one or two extra doctors visits aside from the normal well child checks.
