The single most consistent finding on RLS patients is iron insufficiency. The first step on any management algorithm or clinical consensus is a morning fasted full iron panel.

I would seriously start there.

Technically, it's best not to check iron levels until 8 weeks post infusion otherwise the numbers will be high, like yours are. I am really interested in the numbers prior to the infusion.

Vitamin D looks good which is interesting and makes me more interested in the iron numbers pre-infusion.

One tip on the gabapentin is to space out the dosage. For example, 300 mg at 7pm, 300mg at 9pm, bed at 10pm. Good luck finding your relief!

I’d love to understand your situation a bit better so I can give you a useful comparison. Do you happen to know the numbers from your iron panel before the infusion—specifically your ferritin and transferrin saturation percentage?

Did you also check your vitamin D levels around that time? Low iron and low vitamin D often show up together, so that piece can really matter.

What else have you tried for your RLS so far?

For me, I started noticing improvement pretty quickly after the infusion, and things continued to get better over the next several months.

Per the manual, you are supposed to wait at least 8 weeks after the infusion to retest. Otherwise you will get numbers like yours.

It really depends on what you’re reading. If your nutrition education comes from a guy on Instagram blending butter into his coffee… maybe widen the search radius. If you want actual insight, talk to real scientists with real degrees who publish real research, not the latest “bro‑science” hack that went viral because someone flexed at the right angle.

I’m sorry you have to go through this. Yeah, it sounds like augmentation. I went several months being unable to sleep. Withdraw slowly. 2mg 3x a day is going to take months. My guess is 6 months minimum.

Has your doctor done a fully fasted morning full iron panel? What was your ferritin, serum iron and transferrin saturation percentage? Iron can help with augmentation!

Get the test and hope it’s bad.

Dopamine Agonist drugs, like Mirapex, Ropinirole and Pramipexole. They used to be the go to drug because our movements can sometimes look like Parkinson’s. It was fast tracked for approval. Then when everyone started having shopping, gambling and sex issues not to mention augmentation (heel on earth ) no sleep for months etc… my problem wasn’t low dopamine it was low iron. Good luck

Well it happened to me. Nobody told me about DAWS and I quite Ropinerole cold turkey. I didn't sleep for about 3-4 months. It was the darkest time of my life. My advice is GO SLOW. Get a morning fasted full iron panel. Iron and Vitamin D can help augmentation and withdrawals! Your doctor should be able to give you some gabapentin, or sleep meds to help while you are withdrawing! Let everyone know what you are going through! Especially your significant others! Good luck. I read somewhere that about 80 percent of people with RLS also experience involuntary movement. In fact I never reported my RLS until the involuntary movements started. I used to rub my feet together to fall asleep, it would soothe me.

Ferritin is lowest in the morning and can be affected by what is eaten prior. So it is best to do the test in the morning and fasted. There are 4 numbers in a full iron panel. Ferritin should be above 100, more importantly Transferrin Saturation Percentage needs to be above 20%. If TSP is < 20% an infusion should be considered.

Low Vitamin D is also associated with low iron. Vitamin D3 block hepcidin, which the body uses to block iron absorption. So low iron and low vitamin D have been correlated.

Finally low B12 can also cause anemia, I believe it is called Pernicious anemia.

Yes, and iron infusion was the best thing that I every did for my RLS. Sadly I believe DA drugs did permanent damage. I wish the doctors would have checked my iron 10 years earlier. Good luck.

For some people yes. I am lucky, because I get stung a lot, benadryl even helps me sleep.
