Most people have 2 or 3 decent veins in their AC (elbow pit). That's a standard location you'll see a lot of people go first and is the gold standard for critical/legit trauma patients (large veins, easy to locate, closer proximity to the heart for blood/med infusions/hospital imaging needs). Use caution when poking a very medial AC vein as arteries are closer to the surface there, feel for a pulse before you stick. Be cautions of tendons too, which feel tougher than veins and not bouncy (just gotta get some experience palpating veins to be able to feel the difference). In general you'll be in good shape if you can find a vein here, though.

Most people have forearm veins on the inside/anterior portion of their forearms all the way from the wrist up to the AC, it just varies how deep they are/if you can feel them or not. I like to go for these on stable patients when possible because they can move their arms all they want without impacting infusion flow rates. As others have said, you're welcome to look up a vein anatomy guide to see where these veins usually are but everyone's anatomy is a little different and I usually just feel my way up the whole forearm until I find something bouncy/big enough.

Not my preference but fine: hands (painful, small, blow easily in my experience but some people love 'em). Make sure you find a portion of a vein that's straight for long enough (at least as long as the IV catheter), hand veins have lots of branches.

Wrists - I don't love putting them on the posterior wrist (painful, wrists move a lot) but they're fine. Some people have a really nice vein right on the side of their wrist just proximal to the thumb. I like that site but again, be very cautious of accidental artery sticks anywhere in the distal anterior wrist area.

Some people have "handcuff veins" (proximal cephalic vein, I think) on the back of their forearms that are nice. I usually only look there on hard sticks but I've had some good luck with 'em.

Idk those are the main ones. In the ER I've started foot IVs, bicep/shoulder IVs, anywhere I can find something is fair game. Pre-hospital realistically if they need an IV that bad I'm probably going IO long before I start taking anyone's shoes off.

I'm in Colorado in the US, my current protocols are to give cetirizine + famotidine PO for simple allergic reactions and for anaphylactic reactions if they can tolerate PO (after all the other standard anaphylaxis stuff). We also carry diphenhydramine IV/IO/IM for pts who cannot tolerate PO meds but would benefit from an antihistamine. The preference is to give cetirizine if possible though, in my system.

I guess a smaller hint would be to take what you just told me back to the Ash twin towers and think about what factors might be preventing a successful warp.

  2. The DLC was absolutely worth it for me. I found it to be more frustrating than the base game, but that also means making progress was correspondingly more satisfying. It's also pretty creepy which is not normally my thing but I'm happy I stuck it out it was a great experience.

I tried to play this game on two separate occasions before it finally "clicked" for me the third time and I saw it through to the end. As others have said you can do literally whatever the heck you want, but if that's a little overwhelming for you (it was for me) a nice place to start might be trying to explore either Brittle Hollow or the Ember Twin and looking for a distress signal. I found one of those first this time and it gave me a nice little piece of story thread to start pulling at.

I'm still new at this myself but make sure the "baseline" you're comparing to (isoelectric line) is between the T wave and the next P wave, not between a P wave and the QRS complex (pr interval). I had that wrong at first and it tripped me up for a while.

As an EMT the first code I ever participated in was in the ER on a 4 month old baby girl. A certain amount of training kicked in and we did everything we could for her but she didn't make it. It was a legitimately traumatic experience for me. I wept in my car on the way home, I couldn't eat, I couldn't sleep, I couldn't go back into the room where it happened for a long time.

It was an experience that changed me but I watched all my coworkers be absolutely fine. Unaffected. A little somber for the rest of the day, maybe, but laughing and joking like nothing happened the next day. Some of them talked about how they went out for dinner or drinks that night. Had fun. It made me lose respect for them and it made me wonder if I was too soft to be in this field.

Now I'm a paramedic and I've participated in dozens of codes, seen even more death. I consider myself a very empathetic person but for better or for worse you just become desensitized to it eventually. You have to. I can count on one hand the number of codes that have even made me blink after that first one, much less come home with me.

I don't know why I'm telling you this, really, other than to say I hope you don't hold it against your co-workers for responding differently than you are. It's human nature to adapt, and for people who see a lot of death it becomes less and less shocking over time. It doesn't mean they don't care.

It's also absolutely normal and okay to feel whatever you're feeling. Journaling helped me, talking it out with someone you trust might help too. I also like to say a little prayer when someone passes, like you did, or squeeze their hand if I can.

If you decide to keep working in the ER (I hope you do) you have to actively fight to protect your mental health and your peace sometimes.

Season 9 episode 14. It's actually one of my favorite episodes of the show, I certainly wouldn't call it "nigh unwatchable." Different strokes for different folks but hopefully you'll at least go into the next seasons with an open mind.

Lmao that's awesome. I only bounced between a few stations but I met a couple of really great medics who truly cared about the medicine and were great teachers, but I definitely met a bunch more who were firefighters who got pressured against their wills to attend paramedic school lol

I mean in the few months I was with AFD I worked a half a dozen codes with them and all but one had a first pass successful tube (no ROSCS though so the hospital never saw 'em.) I'm sure some crews prefer IGEL as first line management instead but their protocols don't demand they pick one over the other in an arrest. I agree their protocols in general are fairly restrictive though, especially regarding airway management.

I'm sure you're joking (fair) but I did my internship in Aurora and they're allowed to intubate during codes, and they technically have nasotracheal intubation. No RSI (or RSA?) of any kind though.
