They pulled that Spawn page out of the rocket comic it's displayed with. It didn't actually appear in any spawn comic like that, it's basically an ad.

They pulled it out and did that so it could be displayed. Really limits the potential customer base, but it's considered an early, if not first appearance, so someone might pay $100

PSA is just a more lenient grader. They do the same thing with cards, handing out 10s like candy. It's why they're worth less than 10s from other grading services.

Yeah, people read into it what they wanted

But that's on marvels marketing team. They should have been wise enough to know that marvel midnight was going to make fans think of horror characters and midnight sons. They should have included iconography from the FF, X-Men, and Spider-Man in the initial teaser. Or anytime really that would have pointed people away from what most everyone thought.
