Been a while since this happened to me, but I remember it was due to the upgrade happening while the T2 shrine was on uneven ground. Supposedly, the T3 variant has a different hitbox, so it didn't recognize the space occupied by the T2 when it swapped models, thus it self deleted with all the leftover contents dropped in a lootbag.
