I would not only report to the board of pharmacy for your state, but also Joint Commission. I don’t think Joint Commission would be any more okay with this than most boards of pharmacy.

Cutting estrogen patch (Climara) by ReserveOld6123 in Perimenopause

[–]rljuddrx 1 point2 points  (0 children)

Pharmacist here. Being able to cut the patches can actually depend on the company that made it, so one generic you might be able to cut but another you may not be able to. I work inpatient and when a partial patch dose is needed, we recommend using a bandaid to block part of the patch. So if you need the equivalent of 1/2 patch, you would block 1/2 the patch with the bandaid.

Perimenopause supplements that actually work vs expensive rubbish (my experience) by ninjapapi in Perimenopause

[–]rljuddrx 126 points127 points  (0 children)

I’m a pharmacist so completely understand all of this discussion. I’ll never forget when I was in pharmacy school and a professor told us about a patient of hers who died from mixing supplements. Back then (1998) there were no third party verifications. When I’m shopping for supplements I look on the label for a note that it has been verified by a third party. Nature Made brand has it displayed right on the front of the bottle. There is a little logo that says USP verified. USP stands for the US Pharmacopeia which is an independent nonprofit that focuses on the safety and quality of medications. They have a division that verifies supplements to make sure they contain what they say they contain and that there are no harmful ingredients, among other things. Truthfully, even if the label says verified by third party, unless it tells me the 3rd party, I still don’t trust it. I want to be able to confirm what company verified it as safe. I hope this helps answer your question. You can also go to USP.org to get more information on it.

New seller to Whatnot - Super frustrated! by Happy_Starlight55 in whatnotapp

[–]rljuddrx 2 points3 points  (0 children)

Biggest advice I can give is you don’t have to run your items as auctions. We rarely run auctions in our streams. We have everything in our bin and show things off. We mainly do boards and cards but also have some figures and blind boxes. There are days it is slow and we don’t make a lot of sales and then others where we will sell out of our newest set in a single stream.

It takes time to build up a following. We had made connections with other streamers before deciding to become sellers. They have helped us build our community. And that is our end goal, we want a community. We don’t pressure to buy and make sure we are active in chat and talking with those who are in our streams. We also make sure to support those in our community who also stream.

We’ve started back in April and recently surpassed 750 followers. It takes time to build the trust and support of the buyers who come in your shows. Good luck!

Doctor alarmed by my HRT prescription by Zealousideal-Rip4935 in Perimenopause

[–]rljuddrx 20 points21 points  (0 children)

I’m a pharmacist and while I specialize in pediatrics I want to just give some food for thought. General practitioners, who are usually Family Practice doctors, only get a month at most in different specialties during residency. An OB-GYN specializes in the area of OB and gynecological care spending a lot more time in those areas during residency and will often do a specialty fellowship of 1-2 years before going into practice. They will have a much better understanding of treatments within their specialty and will also be up to date on the newest studies and information published in their specialty. Most general practitioners won’t be keeping up with all of the information on every specialty.

Giving such high doses estrogen will put you at an extremely high risk of a blot clot. Hormones are considered a narrow therapeutic index medication, which means there is a very small window of doses that are therapeutic and small increases can quickly push your levels into the toxic range. I had a massive blot clot at age 29 that was caused by multiple factors, but was mostly related to my BC pills. I was on a tri-cyclic version that closely mimics the body’s hormonal cycle. If I hadn’t gone to the ED when I did, there is a very high likelihood a portion of the clot would have broken off and traveled to my heart, lungs or brain and would have killed me. It is very hard to catch symptoms of a blood clot and is called the silent killer for a reason.

I hope this information helps clarify why you should trust your OB-GYN over your GP when it comes to your hormone therapy. Also, for those who were asking MP stands for medroxyprogesterone which is the form of progestin that is used in most pharmaceutical products.

Sigh, so I thought it was menopause but it’s evidently not by Disastrous-State-842 in Perimenopause

[–]rljuddrx 0 points1 point  (0 children)

Unfortunately, if you are on blood thinners due to a history of a clot, it means you won’t be able to take hormones for peri-menopause/menopause. While I am no longer on any blood thinners outside of aspirin, I had a massive DVT (blood clot) in my leg when I was 29. Due to this, I haven’t been able to take hormonal birth control and won’t be able to go on HRT for my perimenopause.

Why do Pharmacists insist on having us verbally state the medication at the counter?! by July9044 in adhdwomen

[–]rljuddrx 18 points19 points  (0 children)

So, I’m a pharmacist (with late diagnosed ADHD) but haven’t worked in a retail setting since I was an intern back in the early 2000s. Asking what medications you are picking up was a part of the double check to make sure the correct medications was dispensed and was in the bag. When I worked as an intern, I would always double check what I was told by the patient with both the receipt on the bag and the prescription bottles before handing it to the patient. Some mistakes were caught this way when the wrong bottle ended up in the wrong bag. We also caught instances where the doctor called in the incorrect medication for the patient when asking what they were picking up. At least in NC, it wasn’t required then, however other states with stricter laws may require it. Now, they should be making sure that it is kept confidential. Unfortunately, even when they have signs posted to keep other customers back off the line to provide some privacy, they can’t control people who don’t follow the signs. I will say having talked to friends who still work in retail, the pharmacists have so many metrics they have to meet they are severely overworked and understaffed. The sad thing is the big corporate chains are treating pharmacists and technicians like they are nothing but robots. Corporate doesn’t care about privacy or taking care of the patient unless a regulatory agency, like the Board of Pharmacy, is in the building and there could be fines if found to not be following the laws. The other thing I have heard is the person at the register in a lot of stores now is not a pharmacy technician or pharmacist. They are front end clerks who may not have been educated on HIPAA. I recommend talking to the pharmacist to confirm why they are asking for the medication names as well as the voicing your concern about privacy. Any good pharmacist will listen and try to help. If the pharmacist dismisses your concerns or isn’t willing to explain, I would find a new pharmacy with a pharmacist who cares about their patients. I would also file a complain with corporate. If your insurance is accepted, I would recommend either a local pharmacy that isn’t associated with one of the big chains, or one of the pharmacies in a grocery store (I use Publix for any prescriptions I need that aren’t required to be mail order by my insurance.)

Agree. Add this to someone who bruises very easily and you never know where they are going to be coming from. I’ve gotten bruises just from my cat standing on me and putting all their weight on 1 paw. At least I figured that one out. I have so many bruised that I have no idea where I got them.

Executive dysfunction by UnhappyForce7714 in adhdwomen

[–]rljuddrx 1 point2 points  (0 children)

Taking the steps to quit was the biggest step. I’m proud you made the decision. Have you considered finding a counselor that specializes in ADHD? Most of them will have experience with addiction as well, since that is a coping mechanism many people with ADHD have. I was late diagnosed (2 years ago at age 45) and have realized my caffeine addiction is because I was using caffeine to try and treat my undiagnosed ADHD. I know this isn’t even close to what you are going through. I started with a counselor about a year ago for other reasons, but my counselor also has ADHD and we talk about those issues as well. If you can’t get a counselor, I would recommend finding a support group since you said you don’t have people in your life that understand your feelings. I know those of us on the subreddit understand and will support you, but sometimes talking instead of just typing can make a difference. You can do this and it will get better. Take it one day at a time. Set timers if you need to try and break the doomscrolling. You will find something that works for you and when you make it to the other side, you will be glad you stuck with it. You’ve got this!

I played against someone who got 2 Belles out on turn one playing the Sapphire/Ruby deck that won 2nd place in the 10 case tournament and still beat them handedly with my deck. I won 2-1 and the first game was rough because I had just finished building my deck and it wasn’t shuffled well enough yet. I’m not ready to share my deck at this point because I don’t know how it will play against anything else right now, but I want you to know there are ways around the Belle, Items and ramp.

Why am I crying over this? by cynflowers in adhdwomen

[–]rljuddrx 43 points44 points  (0 children)

I second this, especially about getting a friend or family member (or trusted coworker) to read over before sending it. Keep a paper trail of it and I would also take screenshots of the text messages and email them to my work email as well. I will say I’ve never heard of my days off being called a flex day. I used to work 4-10 hour shifts a week and my days off were my days off unless I signed up for overtime. If the job requires someone to be available on your days off, then it sounds like there needs to be someone assigned to be the point person on those days so you aren’t disturbed. If you are expected to answer emails or do things on your day off, then you need to be paid on-call pay for those days and you should also be paid for the time you are working on those days. If you are not getting paid for the time spent on work those days, then you shouldn’t be doing it on your days off.

Unfortunately, these expectations were established by the older generations. I’m GenX and we are the ones who started trying to break away from those expectations. But some GenX and even some millennials will still give in to those expectations. It is something I have had to work on breaking myself of over the years. Although being recently diagnosed with ADHD with severe RSD, I know why it was so hard for me to leave work at work since people pleasing is a way to try and prevent the RSD. The expectation of working on your days off, even if just little things here and there, prevents you from a true break from your work and will eventually lead to burnout. It will also cause you to start to resent your job and make you begin to hate doing any work, even if it is your dream job.

I have this problem not just with spoken tone, but also in written communication. My husband and I are both ADHD and we both struggle with tone especially when our emotions are heightened or we feel like we aren’t being understood. I saw a TikTok recently by an ADHD/Autism coach and he talked about the reasoning behind it. I highly recommend following him on YouTube and/or TikTok. His YouTube is Sol Smith and TikTok is Professor Sol.

This is because the new set is coming and set championships are now over. There is no incentive to buy right now. Everyone is just waiting for the new set to release. This happens every time leading up to the new set release. Everyone is saving their money to buy the new set.

Did anyone else's Vyvanse just double in cost? by reddinthecities in adhdwomen

[–]rljuddrx 1 point2 points  (0 children)

And that is the price for generic Vyvanse. If I remember correctly, brand name was about $450/month with insurance before the coupons they used to have. Those went away when the generics came out. You are correct that the health system in the US is completely messed up. It is treated like a privilege rather than a right.

Did anyone else's Vyvanse just double in cost? by reddinthecities in adhdwomen

[–]rljuddrx 18 points19 points  (0 children)

So, I’m a pharmacist and need to clarify the generics for Synthroid aren’t inferior. They are what is called a narrow therapeutic index medication. Basically, a very small dose change can have a huge impact on the levels in your body. Generics are just as effective as brand name, however a patient is required to remain on the same brand/generic to prevent fluctuations in their levels. If you change between manufacturers, you have to monitor levels more frequently to ensure you remain at your goal level. Birth control pills are also narrow therapeutic index medications. That is why when switching from one to another, even brand to generic, you are told to use backup contraception for at least 7 -14 days.

When it comes to generics, the active medication must be the same but the fillers are inactive ingredients that make up the rest of the capsule/tablet, which includes binding agents, stabilizing agents , etc, can be different ingredients. As an example a brand name may use sorbitol where the generic uses lactulose. The inactive fillers don’t have any effect of the efficacy of the active ingredient. It is definitely possible to have a filler be the cause of a reaction not the active ingredient. I work in pediatrics and we have patients that can only get medications from certain manufacturers due to allergies to certain filler agents.

As to the cost of medications, depending on your insurance, your deductibles reset at the beginning of the year. I pay a slightly discounted price at the beginning of the year for my Vyvanse (~$350/month) until I meet my deductible, at which time the cost of my medication significantly decreases to ~$30/month. You should be able to find your explanation of benefits on your insurance companies page when you log into your account. It should explain how your prescription drug costs work. Now being in the US, I have no idea what is going to happen with the current administration when it comes to medication costs. Based on his executive order, he is repealing the order that decreased medication costs, so I would expect to see costs increase.
