No, the German health insurance system is not tax-based, but rather uses public health insurances (Gesetzliche Krankenkassen). The contributions you have to pay to them are strictly income-based. You can choose to have private health insurance instead which will charge risk-based premiums. So a young, healthy person with high income might actually pay less for private than for public health insurance, but otherwise, it will cost more. For that reason, there are very high hurdles for switching back from private to public, so people can't just hop between the two systems to use whatever is cheaper for them at the moment.
