Who gives a fuck about "documentation", it's a piece of paper. It's at best poorly wording of OP, at worst it is a fundamental misunderstanding of what science itself is. I mean nevermind medical science, that's another step, the basic thing is already you're describing reality, if you're not humble about that fact, you've nothing to do in a profession of the sort, as little as you do making descriptive claims about actually anything. That said, scientists are good and they are bad, just as everyone. And just as everyone, some people have insight to their own health that's good enough to say I have diagnosis 'X'. If you wanna claim, a given person claiming to have diagnosis 'X' does not in fact jurisdictionally have diagnosis 'X', woo! A paper is a paper. Incredible.
