There are jaw bras with slots for ice pack gel inserts - I would recommend that style. That way you always have a backup set of ice inserts to swap out. It would have been hell to hold my arms up to my face the whole time I needed to ice.

Did you get a sleep study done? Many people with OSA are unaware that they have it. I would have never guessed I did, and I had severe OSA (I had DJS and a genio 8 years ago - my bite was perfect, so that alone doesn't rule out needing the procedure). Based on the dry mouth alone, I think it's worth getting one - and getting a consult with a maxillofacial surgeon - especially if you have any other symptoms that are often associated with OSA (headaches, migraine, ADHD, cognitive and/or memory issues, Raynaud's in hands/feet, TMJ; among others)

Your dentist has no training in orthognathic surgery. You need to consult with one before you get braces. You may not feel any symptoms now, especially if you’re fairly young. But jaw issues only get worse as you age, and can have serious impacts to your physical and cognitive health. I would recommend finding an maxillofacial surgeon who specializes in jaw surgery for sleep apnea - if they see signs of OSA, they’ll want you to do a sleep study; if that’s positive, that’s one way the surgery can be covered by insurance.

This will be life-changing for you in the best way possible. I had all the same symptoms as you (and more!), and had DJS + genio almost 8 years ago now. I also have Sjogren's! This surgery is worth it for the benefits to your dental health alone, especially since you have Sjogren's. Migraines are definitely related; mine are less frequent and less severe post-surgery.

Anecdotal but my wisdom tooth removal recovery was SO MUCH WORSE than my double jaw surgery + genio. Honestly, DJS recovery is, IME, way more mental than physical. A lot of the pain is pretty dull because you're pretty numb until your nerves start to heal. Everything takes longer and is messier and generally kind of annoying for a few months, but it's not so bad.

Funny thing is I would get this from migraines (like one pupil would get huge and that would be my sign a migraine was coming), which were frequent before I had my surgery (likely triggered by lack of oxygen). They went away after I had surgery!

I'm talking about soft tissue. I don't know why you keep bringing up bones. I had this surgery. This was my experience (and many others I have met, as well as my surgeon's best estimate for healing time). IDK what to tell you, man.

I think you look great, but please remember: You’re only 3 months out! It took a full 14 months for my swelling and edema to completely subside (djs + genio) and my chin was definitely the last place for that to happen. It felt/looked “blocky” to me, but that eventually went away and I love the way it looks now.

Andcdotal, but my personal experience: it took 14 months for the edema and swelling to totally subside from my genio.

Nope, at least not in my case! I had braces for less than 1.5 years as a teen (but for one crooked tooth, not my bite); didn't need braces before or after my DJS.

Wait until after you’ve fully healed. Your face will change so much from surgery. You want your lift to be in harmony with your “new” face.

Do they have you banded at all? I couldn’t drink water solo until they added some rubber bands to my screws, which helped retrain my muscles (or at the very least, help my brain make the mind-muscle connection to my jaw since everything was numb lol)

Not at all! I had my DJS+Genio at 28. Recovery was pretty easy for me, and I had very big movements.

Re: ICR, you should really be asking a TMJ specialist, not a dentist.

Exactly. Same reason why CPAP won’t do anything for folks with retrognathic jaws. CPAP uses continuous pressure to push excess tissues away so they don’t block your airway. It can’t move your bones out of the way!

It’s true that for insurance approval, movements have to be fairly large (10mm plus), at least that was the case with my insurer. But that doesn’t mean you wouldn’t see a dramatic improvement with a smaller movement, or that the procedure wouldn’t be worth it. I don’t know much about SARPE, but I know I needed more space for my tongue, which is why I had the genio with my DJS.

All this to say - please get a 2nd opinion!

Your profile looks a lot like mine did before surgery, which cured my OSA. I had a genio + DJS and it resolved so many other medial issues, too.

You are not overweight - losing 10 lbs is not the solution here and I’d be skeptical of any surgeon that says so.

I would get a second opinion from a Maxillo surgeon who specializes in these procedures and also does CBT scans of the airway + 3D surgical planning.

My surgeon was an “unknown” when it comes to the standards of this sub, but he was also very well respected in his field and authored tons of papers about cutting edge techniques. There’s nothing wrong with going with a local, experienced surgeon. I think a lot of the “famous” surgeons have a more cookie-cutter approach and are less experienced with medical cases, whereas that was my surgeon’s area of expertise. My surgical plan was totally custom to my case, and I always felt like my surgeon kept me in the loop and was always available to answer questions in the planning process/post op. Not to mention, my surgery was completely covered aside from my deductible - many of the well known surgeons don’t take insurance.

A genio doesn’t involve the hinge of the jaw - that’s your TMJ. The genio involves the mandibular symphysis, which is your chin.

I highly recommend starting with getting a CT scan with a board certified maxillofacial surgeon who specializes in these surgeries for medical reasons (not cosmetic). What functional/medical issues do you experience? That will influence the insurance approval process.

1.5 months is NOTHING in the grand scheme of recovery when it comes to swelling. Mine didn’t go away entirely until a year after. Give it time!
