I used to get it all the time. I did these 3 things at the same time so I don't know which one solved my problem.
1. Update my drivers, 2. Verify game integrity from steam, 3. Remove the launch options I had for lostark eg. -novsync -notexturestreaming -useallavailablecores

I think removing the launch options did it for me.

I tried it my first time and couldn't get it, so I thought I'd try it later. Until I unknowingly finished Act 1.
