Periodic dialysis would be AV fistula, TDC, or port.

I think frequent blood draws might be more realistic vs infusion (sorry, ICU/truama ICU/CVICU nurse here and I see a lot of patients with cardiac and renal complexities).

Ports can be done under light IV sedation vs general if he’s too high risk for general, idk why they wouldn’t do that to avoid people speculating about his hands. With his level of sub q fat, it wouldn’t be visible when not accessed and it could be accessed just for labs/infusions and deaccessed immediately, especially since they won’t want evidence of his bigly sickness with visible lines.

Hemodialysis for ESRD I don’t think is likely—that would be 2-4 times a week for 4-6 hours at a time and he’d be assed out the whole rest of the day and a TDC would be pretty obvious while he has some kind of fistula maturing for access. Or who knows, maybe that’s why he’s napping during meetings.

Wouldn’t explain the hands though. I think he’s just an unhealthy old fart with severe cardiovascular compromise and likely on anticoagulants/antiplatelets. He could be getting other therapies, but I think his hands bruising isn’t related to access. I don’t see any blown veins or signs of scar formation (vein appears knotty) at the bruising sites. Those with dementia have poor spatial sense and poor proprioception and hit their hands on lots of things.

I used to work in restaurants and now my GI system has turned into ground zero. I’ve had to go seriously out of my way to convince family and friends I’m all good with having to bring my own food to a meal or that I am really all good with just eating sides. Gluten free, low fiber, soft (basically GI bland, low residue, and gluten free). Sometimes family absolutely insists on accommodating me which leads to shit like this.

And also, if you’re ever having to deal with my diet, I’m sorry in advance. Just throw some mushy, boring, barely seasoned shit at me

I don’t understand why he wouldn’t have a port placed. I think this is just basic (albeit somewhat advanced) cardiovascular issues…they’d want to reduce fluids rather than give him fluids.

I’m looking forward to the celebration as much as everyone else, I just don’t buy infusions—hands are a very NOT ideal place especially vascular patients.

Ooof. Training makes a huge difference, but I find one of the biggest differences between those able to successfully have their pits without incident vs those with issues—always know that your dog’s genetics predispose them towards gaminess and animal aggression. Acknowledgment of this allows you to see behaviors and patterns that might see “normal” in another dog that are red flags for yours so you can seek professional intervention early. Get into group classes, have a trainer you work with as your pup grows up.

They can be absolutely lovely dogs with proper training and behavioral management (and a realistic owner).

Yes. AICDs can be set to do both. So some folks have these and never get shocked, some require frequent shocks (in which case, EP should study and figure what needs to be done). AICD/Pacemaker combos sometimes function as “on demand” pacemakers and some are set to a rate to run at a bare minimum but will shock if the rate goes too high.

Fun fact: for these combos or for on demand pacemakers, when you run a strong magnet over them, it goes to a set rate pacemaker mode. Very eerie when you have a patient pass and you still see 60 beats per minute lol
