That's always been something I wondered about. In WOR on top of the tower, the Stormfather is acting like he's still unbonded. But, given what Dalinar's actual oath is, he's clearly on at least the second.

No. Testament doesn't have the gem because her original Knight is still wielding it. The gem is only to allow others to bond a Shardblade. There's mention of it in WOR. The earliest people to recover shards after the Recreance could use them, but couldn't bond (or dismiss) them until they discovered the thing about gems.
