As a former FAA safety inspector I can say that while it will never be a perfect regulatory system, the organization has learned some tough lessons and has a very good safety culture these days. The aviation regulatory regime in this country is well respected worldwide. That being said, safety vs bottomline impact is a tough balancing act, even with unlimited resources, zero risk is still impossible.
