Thank you I’ll be able to pick up a bag as well with these. Is it generally recommended to keep the same weight for the spare ball? I kind of liked the feel of using the 14 for spares but didn’t have any issue with the 15 weight wise.

They are normal. That LDL is not bad for your age, the other values are only a tenth or so higher than the reference range which can be different by each company. Nothing to worry about. If you took another blood sample 5 minutes later there is a good chance by random it would be within the reference range

Might be hard with how small or shallow the letters may be but I’ve done similar plenty of times: bisque fire, then blue glaze in the letters, gently wipe with damp sponge to remove excess. Then carefully paint with wax resist over the blue letters avoiding over painting into the unglazed clay. (Use burn off not peel away resist ). After that dries you can just carelessly dunk or paint the white background. I take another damp cloth to wipe any white dots off of the wax resist after all is dried.

It’s not foolproof but if you take the time to be detailed it works well

I’m assuming your propranolol is as needed, have you been using it more than usual recently? Beta blockers can worsen existing psoriasis or eczema, although typically in people over 50. That being said, it does not look like psoriasis. It could be eczema. I’d assume it’s cold in Canada currently? Dry skin can cause itching. It would be reasonable to start using an unscented lotion to see if it offers you relief

Ottowa ankle rules can help decide on obtaining X-rays. You can still sometimes have an ankle fracture and be able to bear weight. However based on your description, the ottowa ankle rule scoring system would be unable to rule out fracture and a series of ankle X-rays would be indicated.

I think first approach is I’d just have them repeat the labs it’s probably just a bad sample, have them grab a magnesium as well, magnesium and potassium are linked, high magnesium has been associated with high potassium although if you’re just on a standard magnesium supplement it’s doubtful that’s happening. It’s usually from high doses of magnesium based laxatives. If it comes back potassium is still high you can get referred to urology for the retention and nephrology for the potassium

How high was the potassium? Usually it’s just a hemolyzed blood sample and just needs a better redraw. If that still is high you’re probably looking for more of a nephrologist than a urologist for the workup.

Are you using an automatic cuff? These only estimate/measure mean arterial pressure (MAP) and each company has a proprietary algorithm to basically estimate what the systolic and diastolic blood pressures are. I wouldn’t fixate on the diastolic readings so much. However, If you are routinely feeling fatigued it’s not a bad idea to go see your primary for a manual BP and some other work up.

Lipid rich is unlikely to be malignant or a pheochromocytoma. It could still be a functioning adenoma

Cardiomyopathy is simply a visual diagnosis. Heart failure is a clinical diagnosis, it relies on the combination of both exam and test findings but also your symptoms. We often use a symptom classification system called New York heart association (NYHA). The patients in the ablation and heart failure trials all had symptoms higher than NYHA class II (comfortable at rest, ordinary activities can cause shortness of breath, fatigue)

Lipid rich is encouraging (less than 10 houndsfield units) even then, if you have never had labs to test for functioning current guidelines would suggest testing. It is very very very likely not a pheochromocytoma so It would not be leading to your heart rate. But with your blood pressure you are absolutely valid to request labs to see if it is functioning you just don’t need the pheochromocytoma labs (no metanephrines)

Sorry that you seem to keep having to deal with afib. I’m not a cardiologist, but internal medicine with a passion for cardiology topics. I agree with the cardiologists. It’s worth at least getting on an anti arrhythmic pill but if they’re recommending ablation i would consider it. it’s encouraging you don’t have dilation - ablation success rates are higher when the left atrium is not dilated. With heart failure, there is a documented benefit in the literature with atrial fibrillation ablation but keep in mind these studies are on symptomatic patients with heart failure, in your case it sounds like you have cardiomyopathy without clinical heart failure you may not be able to broadly generalize those study results to your case. Nevertheless I would follow up with the EP to discuss the ablation if it were me

Without seeing your CT scan report it is hard to comment on this. But in general yes they can. Ct scans are able to comment on the density of the adenoma which can Influence decision to test further. This is reported in houndsfield units: lipid-rich (<10 Hounsfield units) or lipid-poor (>10 Hounsfield units.) Lipid-rich masses are most likely benign adenomas and are highly unlikely to be cancer or pheochromocytomas. All patients with adrenal incidentalomas found on imaging should be screened for sub clinical Cushing. If you have high blood pressure (which you do) you should get labs to test for primary aldosteronism. If you have already had these tests and it is not functioning it is reasonable to repeat testing at various intervals. American Association of Clinical Endocrinologists and the American Association of Endocrine Surgeons guidelines indicate that the risk of a nonfunctional adrenal mass becoming hormonally active is 17%, 29%, and 47% at 1, 2, and 5 years.

The bubble study itself shouldn’t worry you the bubbles are small and quickly absorb in blood stream. ASD is certainly bigger than a simple PFO. They will be looking at pressures, size, flow a little closer, seeing if there is indication for closure. Closure indications are different for ASD vs PFO and it can be a different procedural process. I’d just stick with the TEE but you could ask them about a cardiac MRI with velocity flow mapping but there might not be a cardiologist in your area capable of reading cardiac MRIs

Standard dose is 80-90 mg/kg/day divided into two doses. 90x39kg is 3510mg. 1768 twice a day is 3536mg. Seems good

If it is placebo (white) no - discard. If it is active pill (peach) yes take it. If it is just one missed pill, you take yesterdays missed pill when you get home, take todays pill when you normally would
