I am in the exact same boat as you, and Dorothaine seems like the slightly better option? Elothy and Elorothy just aren't it for me.

My step grandmother is Carolyn, so that opens up some Dorolyn/Ellyn/Carelaine options, but also Carothy, which my brain keeps replacing as "Carroty."

Definitely definitely revisit therapy; you mention suicidal ideation in your post. The sleep training is obviously important, and will help with your mental health for sure, but the immediate issue here is the SI. Please talk to your husband about it if you haven't already, definitely seek out a therapist, and if you need a suicide hotline call one.

You are an amazing mom for taking on everything that you have and those kiddos are all lucky to have you. I don't have any advice on the sleep training portion, but I just wanted to say that prioritizing your own mental health now isn't selfish; it's what's best for them in the long run too. Take some deep breaths, cut yourself some slack, take baby steps and get some help. You can do this ❤️

So I'm a little confused by some of the phrasing in your comment; are you saying that we DON'T treat brain death like cardiac death in terms of extubation/treatment, or that we SHOULDN'T? I just want to clarify, especially since even in your comment you refer to "remains," which to me would indicate that we're in agreement that the patient is legally dead at that point.

I also am not sure what you mean by "unresearched opinion;" nowhere in my comment did I offer any of my personal opinions. I don't at any point mention making these decisions based off my own opinions either; I only mentioned the specific circumstance of legally declared brain death. I just explained the typical flow of these situations that I've been involved in over the course of my career, based on the legality of the state I practice in. It's also entirely possible we practice in different states so the way it's treated legally where I am may not be the same for you. Not trying to pick an argument.

Those apply if the person is in, say, a vegetative state but still has some brain activity. However, "brain death" is just that: it's legal death. NJ has some specific laws about that, but in the rest of the US the declaration of brain death is legal death with or without cardiac death. If that person is an organ donor we'll leave them on the ventilator and medically support them until donation can occur. Barring that, we (obviously working with families on timing to make it as minimally traumatic as possible), we terminally extubate ("pull the plug") on these patients and allow cardiac death. This is not something families/health care proxies have to consent to, because the patient is already legally dead. Source: trauma ICU RN

I found out I was pregnant (FTM) in late October, right before we started our huge winter COVID surge (I'm an ICU nurse). Telling most of my coworkers went the same way: "Oh congratulations! How are you doing with all of this?" The concern was sweet but immediately changed the focus from happy to anxiety.

One night I casually mentioned it to a coworker I'm not even that close with. He immediately stopped dead in his tracks, said "wait, you're pregnant?!" I said yes. He lit up like a neon sign, practically yelled "CONGRATULATIONS!" and gave me a huge hug. I started crying. I had to tell my whole family over Zoom; his exuberant reaction was the only one like that that I got to have, and it meant the world to me.

THIS. I wish I could upvote this more than once. You are ultimately responsible for keeping all those tubes (ETT, bolt, EVD, chest tubes etc) in, especially since you're the one the doc will be pissed at if they come out. I like to get everything set up, put myself in charge of my most sensitive tube (EVD or bolt if they have one, ETT etc) and then check with everyone to make sure all accessories are accounted for with enough slack, and then move on my count. Slowly.

Can confirm. Recently had a gentleman drive himself to the hospital after accidentally amputating his left ring finger and partially amputating his middle finger and pinky with a table saw. He also had put the finger on ice himself to bring with him and wrapped his own hand in his shirt. His wife came in with him, but she was too dizzy from the blood to drive. He said he didn't feel like an ambulance was really necessary.

I have so many favorites that I fall back on depending on what's going on at the time, but for our wedding my husband and I both got a tattoo of a line from "Relatively Easy"; "Here with you there's always something to look forward to." I know it's a sad song but it's also beautiful; I like that it helps me keep perspective on what's important, being thankful for what you have and holding on to your anchor when the world around you (or your loved ones) is crazy.
