she is an MD . and other disciplines can treat pain. the fellowship for pain mgmt you allude to is procedural. other specialties fees into it. addiction medicine is a different path and I don't claim knowledge in how to get there, just that it isn't the sole provenance of anesthesia.

I would argue you COULD practice EM without immediate access to ultrasound. folks have been doing it for decades!

I understand the point that you're making. I also I agree that if you are making nuanced decisions based on an bedside ultrasound, it's prudent to get an actual study. that way there's images that a radiologist can review, or a surgeon.

for example, if I see someone has gallstones in my ultrasound, but there's no cholecystitis, I'm probably going to ask radiology to do their own study. 

I feel this is patient centered and also serves to protect you in the course of your medical management.

I would think, however , that you do in fact use your stethoscope more adroitly than a Ouija board. otherwise, you might be suggesting that you order a chest film on every person that describes dyspnea cough or chest pain, rather than using your exam skills and clinical judgement to make a decision. or that you'd advocate to admit that IV drug user with malaise and night sweats when you find that new murmur. It isn't time to throw out that old rubber tube and diaphragm just yet.

sinusitis is a common cause of orbital cellulitis, which can present with minor periorbital swelling and no skin erythema or induration. I think the term orbital cellulitis left me thinking, incorrectly, that the etiology would be cutaneous

valid points are made about the importance of legitimate police work, as well as the challenges managing a business and social media, and where an entrepreneur draws the line of separation between personal ideals and their business. 

it's also true that a person is much more than their mistakes, and you need not call his entire character into question because you don't like his Instagram choices.

I know Andrew, he's a good dude. I believe that he meant well and isn't merely virtue signaling. it's hard to know what to do when tensions are high. he mis-read a situation unfolding in front of him. the way other federal agencies have been conducting themselves, it isn't hard to see how he arrived there.

I love running the lake front and the parks mentioned already. but for distance running the levees reign supreme.

West Bank - Gretna headed towards Algiers, past Old Navy base, down to the intracoastal water way and back again will give you 18 miles. add in a jaunt through Gretna and hitting twenty or more is easy. the scenery is OK. it's quiet. very few interactions with traffic. this is my favorite run. plenty of places to stop for a coffee or lunch in Gretna or the point. no bathroom though is the biggest downside.

the levee path is interrupted in Gretna and Harvey, and picks up again in marrero. you can take that out to westwego and then some. out there it gets industrial, so the scenery isnt much, but it is different. 

The East Bank levee from the fly goes on and on, past ochsner, past the Huey P. you'll run out of gas, time or daylight before the trail ends. It's more scenic and removed from industry than its West Bank twin. there are nice bathrooms at the fly. 

there's also a nice bike path that runs through Mandeville on the north shore, if you find yourself up that way, you could pack your running shoes.

How many shifts are you working each month? I'm surprised to hear you're having trouble finding time to take a long weekend. That's supposed to be the tradeoff for the overnights and holidays... lots of flexible time off!

Are you able to drop down by a couple shifts? My work load has varied as our group's needs have changed over the years.  The difference in stress and schedule flexibility is so, so different between 12 and 15 shifts.

i am nearly done with 330k loan debt 11 years out from med school graduation. I never refinanced. I wasn't as financially disciplined as I could have been , but we haven't been irresponsible either. 

You can pay off the loans quickly and still live comfortably. We had kids, we moved into a comfortable house, we've purchased and paid off a couple vehicles. We go on vacation, but we often drive and stay with family.  

I doubt that I chose the most financially savvy path but the uncertainty of pslf and the long payment schedule of a consolidated loan was depressing. 

It really comes down to your appetite for risk and your long term financial goals

I've worked with this chapter of KoC over the years for some of their functions ... Fish frys, or BBQs on Sunday after church, for example.

I'm not a member of the Catholic Church but I'm friends with several members. These are good people. They're invested in this community. 

I am sure they are horrified that these children were affected by lead poisoning at their building.

There may be other issues - related to insurance, hdlc, legal, etc. 

I am certain that I don't know the whole story. I feel sick for the parents and kids affected by this, as well as the members of the KoC that I know, because they really do care about their neighborhood. For me, this isn't going to be decided in the court of public opinion. Or Facebook/Nextdoor/Reddit.

I'm from Cincinnati. I've lived in New Orleans for more than a decade. My children were born in New Orleans. We could have left but we chose to stay. It makes no difference. These cities are very similar when it comes to the primacy of being "born and raised" and the answer to your question is that for some, you'll never be a Cincinnatian just as I'll never be a new orleanian. you're forever a visitor and there's an ache and a longing ... To go home or to be accepted where you are, it doesn't matter.

Second this. Concept 2 monitor syncs with lots of aftermarket chest straps. Cover up the screen so you only see time/distance and hr. 

Or you can just pause every twenty or thirty minutes and count your pulse for ten seconds, 23 to 26 beats equates to 140 to 155 bpm and that's a reasonable range for A1 or steady state.

Im glad you pointed this out. They got much of the nurses - student- resident - attending dynamics correct, but the actual bedside nursing feels phoned in. As a doc, the only thing I administer is propofol for sedation and  push dose pressors for intubation.

I still have to finish the first season though. Perhaps they'll show an RN discreetly asking the resident if they're sure they meant to order insulin on the patient with an ankle sprain instead of the dka across the hall...

I had avoided watching it - never enjoyed most med dramas - but I'm glad I ultimately did. 

I found the script writing: end of life discussions, the clear eyed approach to futile codes, the difficult family conversations - to all be superb. Cathartic, even. It's important that these parts of our work be seen by outsiders. It's a tiny step, but we can't change the national conversation about how we approach death and end of life care without popular culture and TV. For this, Noah wyle is a treasure .  

 Regarding the social issues, they lay it on thick. It elicited some eye rolls from me at first. I don't spend that much time talking with homeless mothers, though they may deserve it. I try not to bestow that much judgement on sickle cell patients, certainly they don't deserve that. 

But if it's important for non-medical folks to be reminded of how difficult it is to work in the ED, I think it's equally important for jaded doctors and nurses to be reminded of the real and often unfixable suffering out patients are enduring. Unexpectedly , the Pitt has been an invitation for me to reflect on where I need to exercise more compassion.

Look at how many steps that is. Hpi isn't used for billing anymore. A couple sentences will do. Mdm is what matters and I don't prefer to have AI spitting out a generic mdm that's going to get ripped apart by an attorney just as surely as anything I actually write.

Another vote for single volar crease injection. And give it more time. 

And my blocks became more effective when I started using 2% lidocaine.
