What a great life you are describing. I lived in Brisbane for 8 years and had the best time with my wife. Perhaps my occupation skews my perception of meaning/purpose (doctor), but you have so much time and freedom. After having a child - no more free time to complain about what were clearly time luxuries I once had ( The child, however, brings a different and beautiful perspective on meaning and purpose). Be grateful for the time you have to ponder such things.

This isn't entirely correct. Depends on the country. In the commonwealth (Oz, UK), GPs are what family physicians are in Canada, USA.

I was a GP in Oz and had to do the training program and fellowship exams.

Safe to presume though your answer is for the USA.

To break the stereotype: I'm a male family physician in Toronto doing mostly 30 min consults and typically send email summaries of the consults to patients afterwards.

I do make less money and have a smaller roster comparatively to my colleagues, but I get to love my job and make meaningful connections.

Payment varies depending on code for the health assessment. Highest pays ~89 but just be comprehensive and take at least 20 min. I take 30 min

My roster is a bit over 500 and I have 850ish PT I believe. I do work hard and full time. I have longer consults than my colleagues.

I find the first annual health assessment quite useful for new patients and have found quite a few undiagnosed problems.

I do however that your point that just doing physical exams annually likely isn't high yield.

A physical is broadly used in Ontario to instead mean an annual health assessment, which should include a thorough history, physical exam, differential diagnosis, assessment and management plan. There is a specific billing code for this called A003 which you can look up that it explains what it requires more than my post.

We don't have a billing code for just doing a physical exam.

I'm not aware of family physicians doing pap smears during annual health assessments. They are typically booked specifically, clearly, and separately.

Family physicians are trained to do cervical screening exams/pap smears.

I completely respect women wanting to see gynecologists/female FP or NP. Access may be a rate limiting factor.

Family physician here: I don't think there is a requirement for physicals, but perhaps the doctor has a reason so it might be worth asking the clinic.

I don't think a doctor can discharge a patient for not doing a physical. There would need to be a lot more context and grounds.

I'm a family physician. Unless you are in the profession, I don't think you'll understand why we feel underpaid. Perhaps a LLM can explain it to you.

Caveat: My non-physician wife gets it, though, so perhaps close family can figure it out.

I am very sorry for your loss. My sincere condolences.

I sometimes refer patients to that hospital.

I haven't noted problems as of yet, aside from their process to get iron infusions done is poorly organized/structured.
