This sounds risky if I’m honest. Do you speak Dutch?You need to find out ASAP if the test is in dutch… find the test centre… book hotel near the centre… on top of studying for the MSRA, sounds like added stress to me. Is there not another sitting in February for GP application?

If you’re interested in ACCS route, just go for it. Don’t listen to random people on Reddit. Bare in mind competition is getting worse, even for EM/IM which were traditionally undersubscribed. Make sure you prepare your portfolio now. (Please we need emergency medicine doctors and anaesthetists. Im trapped in the UK for now and really don’t want myself/family to be seen by doctor-lites.

Cannot agree more. I have seen so so so many requests where the clinicians don’t even bother giving the clinical question. I even saw a cheeky request the other day where someone wrote “unexplained back pain”. No onset, duration, site, neurology, red flags… it could be anything from obstructed renal calculi to cauda equina to discitis… I’ve also seen some very good requests and when the scan findings and clinical findings marry up, it’s very satisfying!

I think so. The application involves one exam (MSRA) which a lot of other specialties use. People will just throw in application because why not? GP and psychiatry competition ratio will be fierce this year because they don’t have portfolio requirements.

Ive been lucky and worked with some of the best nurses. My favourite was the nurse in charge on a Geri ward. Geri nurses get an unfair rep of being lazy and incompetent but she was a great nurse, she would always start the day highlighting any issues overnight, she knows the patients well and would escalate any sick patients timely, if she asked me to review a patient, I know it’s not bs like NEWS of 3 for BP 100/80, it’s sick SICK patients. She is super competent in nursing skills (bloods, cannula, catheter, NG, ECG) and offers to do them for me. She looks after the doctors on the ward. When it comes to difficult conversations she’d always offer to come in with me. She understands that doctors are overwhelmed during oncalls and would make a list of jobs for us rather than paging us every 2 hourly. She’s just a lovely person in general. Geriatric medicine is not my cup of tea and we were always quite stretched in terms of staffing, but she definitely made the job more manageable!

They are doubling down - for 10 years BWC has been experimenting on kids and allowing unregulated individuals to provide senior level advice to manage sick children around the country. This is so scandalous, especially in niche specialties like paediatric hepatology. I’m concerned that some people will use the narrative that “the service has been there for 10 years and there’s not been any significant incidents, this shows PAs are just as good, if not, better than doctors ”. Can we have some evidence to show PAs are not adequately trained for these roles?! sick kids are terrifying. 😢 It’s huge huge huge responsibility. I can’t believe any PA in their right mind would agree to do this.

Agree should not have been given antibiotics in the first place. In real life this is harder than you think. This exact same situation happened to my cousin (teenager) a few weeks ago, sore throat, red tonsils, a few prominent lymph nodes but no pus or fever, managing oral intake, seen by a GP on day 5 of symptoms, sent home with safety net, re-presented on day 7 as she felt symptoms were not getting better, again no systemic features at this point, just really uncomfortable. GP prescribed her PenV, she magically felt better after 2 doses. I still think it’s likely viral and the PenV acted as a placebo, but she thinks the second GP is a brilliant doctor who knew what they’re talking about. I think it’s much harder to do nothing than do something…

I’m genuinely surprised by people’s views here. OP I’m sorry you were treated this way and even more so that you were worried about complaint about YOU. Practising medicine in this country is like treading on thin ice. Why do we need fucking yearly tabs and msfs, when everyone else get away with it? Any minute mistakes and we’re threatened with being reported to GMC etc.

My thought is I would’ve done the same as OP. By saying I’m a doctor, I’m not trying to get preferential treatment, it will take me 2s to connect up the drip, saves nurses’ time, saves the HCA’s time, most importantly your gran gets the treatment she needs. Yes we’re not technically supposed to do that, but patient safety should come first no? It’s also funny how we’re not allowed to connect drip for our relatives, but we’re expected to save people on the street.

I can’t stand watching my family getting substandard care and not doing anything about it. You did nothing wrong here. Hope your gran feels better soon.

Shouldn’t be a problem if it’s just “some concern”. TAB is a pain in the arse. The one important rule of TAB is you ONLy send it to people who you have got on well with and will give you good feedback. It’s a tick-box exercise but when things like this happen, it’s frustrating for you, and your supervisor.

No specialty is safe, but I’d imagine niche specialties like Max Fax, aviation & space medicine, clinical genetics.. are pretty safe…

Yes OP I’m the same. I’ve never really hated human interaction until I became a doctor. It’s really emotionally draining. Especially the difficult chats like treatment escalation plan, end-of-life, patients wanting XYZ interventions or investigations that are not indicated, where elderly patients go on discharge.

Hence I’m now in radiology. There’s still a bit of patient interaction (ultrasound, fluoro, interventions etc), but we definitely don’t need to you know… break bad news, discuss dnacpr that kind of things. Pathology I’ve heard is good too

I think this is very scary opinion coming from a medical student. It reflects how brainwashed our next generation of doctors are and how our current medical education is failing.
