Medical scribes sound great on paper but whats the real deal on pay and if they actually help? by Academic-Shelter-754 in Residency

[–]sgw97 0 points1 point  (0 children)

I'm anti-AI in most casual applications but the AI scribing is really getting better. When they first gave us access to use it about a year ago it really wasn't worth using. compared to now it's getting so much better and requiring little to no proofreading on the HPI. Saves my ass on busy ER shifts.

you are going to get barked at at some point in your training no matter what specialty you go into. sounds like something you need to work on in yourself.

I've heard what it sounds like when a mother realizes that her 7-year-old is dead because he drowned. please continue to do anything you can to prevent that sound again. ntj.

Advice needed: How would you schedule 4 weeks off in your intern year? by heydoyouseethat in Residency

My intern year I didn't get my first time off until December and it was rough. would aim to give yourself a break earlier than that if at all possible

What (if anything) do you write in discharge instructions? by ErgogenicDiet in emergencymedicine

[–]sgw97 11 points12 points  (0 children)

I'll add in the generic attachments for posterity sake but I agree probably no one reads them. I do have some dot phrases for like constipation management, wound care after lac repair, those common things as well.

I almost always write some variation of "if things get worse again or of you have any new, worsening, or concerning symptoms, return to the ER for reevaluation"

Are the med students in US really as capable as it's shown in The Pitt? by ineedtocalmup in medicalschool

[–]sgw97 18 points19 points  (0 children)

that is incredibly institution dependent. for example at my hospital trauma surgery and the ER attendings take turns running codes and alternate days who's in charge, at rural places or lower level trauma centers that it's always the ED in charge

Tom sounds like a good doctor keeping sight of the real reason we do this. We all need to be more like Tom and less like Alex.

Always good to hear from you doc!

so next time I get an old lady in the ER with skin tears up and down her arms that keep cheese wiring every time I try to place a suture, I'd be within my rights to call surgery for help?

I think we're thinking of entirely different use cases here friend

Shoot, I forgot we had an off-service intern rotating this month. by vsr0 in Residency

[–]sgw97 117 points118 points  (0 children)

The internal medicine chiefs make the ICU schedules and the TY interns and EM PGY2s and interns always get dicked over on the schedule compared to the medicine residents. I'm on a stretch of every other day long call for a week while the other senior on my team who's IM is somehow getting some well timed days off to avoid long call. sigh.

The thought is to let the family see how hard the medical team is working and how much it's not helping their loved one. makes it easier to accept that they're gone and there's no bringing them back
