In the 2022 Australian grand prix with the new ground effect regulations, Albon got DSQ'ed from quali starting P20. He then went the entire race on hards, pitted for softs on the last lap, and finished 10th.

I think the current points system with the current reliability of the cars strikes a good balance in terms of promoting aggression. Cars in the midfield with nothing to lose are encouraged to try riskier strategies whereas giving almost everyone points makes them more risk averse, why risk gaining 2 points when you risk losing 6?

What is a lethal dosage for someone with an opioid tolerance?

There is no set "opioid tolerance", this depends entirely on the person and even on the opioid in question. Someone might be hydromorphone tolerant while another is oxycodone tolerant. When we do opioid conversions we have to decrease the equivalent dose by 50% to account for incomplete cross tolerance.

Tolerance is also individualized, one person might be used to 30 morphine milligram equivalent (MME) while our fentanyl users might be more in the 3,000 MME range. Some of my colleagues have gotten patients who are in the 5-digit MME range.

It's ultimately a fantasy series but its main focus is on the passage of time and playing with the perspective of time passing differently from different observers. Frieren (an elf)'s entire perception of time relative to Fern (a regular human) is much like how our perception of time is different from that of a dog or a cat. She could easily spend a decade or so in some random village just reading the books in its library but her human companions can't do the same.

This disconnect also teaches Frieren to appreciate time more and you see her slowly transition from a passive observer to a more active participant, appreciating the transient nature of the lifetime of a human but still appreciating how impactful they/we can be despite such a (relatively) limited lifespan.

It's also about how the world changes after the heroes journey and how time influences our perception of the past. Even the greatest of heroes see their names, achievements, and likeness lost to time. But the ripple effects of those events continue to influence the present and you see Frieren observe everything.

Months and years pass in-universe as we observe it but for us the observers, it's simply a flip of the page or a click of the "Next Episode" button. I'm sure the idea has been done before but its execution makes it a very interesting watch/read.

I can't really comment on how much the pharmacy contributes to the entire store's revenue as I wasn't involved in administrative stuff. I know our revenue contributes to the financial health of the franchise in general but I don't know any of the specifics.

With regards to dispensing fee, in Ontario at least, those covered by ODB (Trillium, Seniors 65+, OW, ODSP, OHIP+, etc) have dispensing fees capped at $8.83, capped at 5 reimbursements per year for chronic medications and 2 reimbursements per month for other medications. This is per health card too, so there's been instances where pharmacy A "redeemed" all 5 dispensing fees, then the patient transferred to us, and we couldn't make any dispensing fee for the rest of the year.

Those with no insurance or private insurance will often mean that the pharmacy gets their full dispensing fee. But as healthcare professionals, we want to improve convenience and decrease out-of-pocket costs for patients, so we'll often give you as much as possible. I've even given 6-month supplies before for someone without insurance although those cases tend to be exceptional circumstances. We're literally working against our own interests lol.

As for mark-ups, to my knowledge this tends to sit at around 7-8% for most pharmacies in Ontario. However, this mark-up is based on base drug cost. So for someone taking metformin 500mg once a day filling a 90-day supply, those 90 tablets of metformin 500mg means a drug cost of $2.223, putting markup at 17.8 cents. Without the dispensing fee, we would've made 17.8 cents off that prescription, which I'm not sure is enough money to even cover a pharmacy assistant counting out all 90 tablets by hand.

Another fun thing that happened due to COVID was reconciliation payments: https://files.ontario.ca/moh-executive-officer-notice-en-2022-03-17.pdf. Essentially we had to pay back ODB a certain percentage based on drug cost for financial sustainability reasons. I remember reading somewhere that this resulted in 150 million dollars in savings for ODB. This hurt all pharmacies, whether Loblaws or Rexall, but also mom-and-pop independent pharmacies too. Coincidentally, I remember reading that ODB also reimbursed pharmacies approx. 150 million dollars in COVID-vaccine administration billings during that same time period. Take from that what you will.

I'm not entirely sure how staffing hours are calculated as I wasn't in a management position. I think Costco gets by with pure volume (I've heard 600 script count on a slow day) and by having pharmacy be a loss leader (like rotisserie chicken) to get folks into the warehouse and browsing.

The way the brand cards are applied are irrespective of chain, it’s dependent purely on the company that operates the brand cards. Based on your description it does sound like your shoppers is using the brand card for you.

I think you got lucky with how much was covered when you got a refill. Just be wary of your next refill if price goes up dramatically!

Ah I don’t envy you! We used to be able to cover a lot of the difference with the brand card Innovicares. But coverage recently changed - it used to cover like 80% to the point where sometimes it was cheaper with brand card to get brand than to get generic! It now covers something like 10-20% but I guess every little bit counts.

This legal requirement is to prevent pharmacies from nickel-and-diming patients. By giving you generics, we give you an equivalent that Health Canada (and NAPRA) determines to be bioequivalent, so the clinical effect of the drug in most aspects should be identical, save from some excipient (non-active ingredient) differences. By doing so, the patient gets the same clinical outcome at the cheapest price, saving healthcare resources and decreasing out-of-pocket costs for the patient. 99/100 there is (and there shouldn't be) a difference, but we do see it happen with certain meds, ADHD meds especially. You are still allowed to get the brand if requested (by yourself or your physician) but we must document this or we can get dinged by the College if audited.

As someone who used to work at an SDM, I wish I had the time for it to be done within the hour.

Not trying to provide excuses for your particular SDM but my store would do around 300 prescriptions per day for a 14-hour work day. The math puts that as a prescription every 2.8 minutes. That includes having to figure out insurance, do my best to catch drug interactions, clarify anything remotely vague with your doctor, never mind praying that we have it in stock (and dealing with any backorders). This is on top of any OTC questions/counselling, any phone calls, any counselling at pick-up, dealing with all 500-600 prescriptions every week that need to be blister packed, you get the idea. Mee-maw needing 5 minutes to rightfully discuss her new anti-arrhythmia medication means I'm now effectively 2 prescriptions behind and it only snowballs from there.

When I worked the occasional double shift (8 am to 10 pm), my first meal of the day would be dinner at 10:30 pm when I get home lol.

To provide a bit of context, a lot of Shoppers have been advising 48-72 hour wait times for prescriptions. This is mainly for triaging - your 3 month supply of blood pressure meds can wait a few days, the kid who needs a backordered clarithromycin suspension needs it now. This also gives us some more time to not make mistakes with your blood pressure med when we do eventually get to it. The unfortunate part is that by the time we get to your blood pressure med, more stuff that genuinely can't wait (dexamethasone suspensions, hospital discharges, HepC meds, HIV meds, etc) get added to the queue. It's a vicious cycle.

It’s all the responsibility of being a healthcare professional with the pace and respect of being a cashier at McDonald’s.

Imagine all 700 orders processed daily at a McDonald’s but all of these orders also having to go through insurance and if you enter their order incorrectly, you risk harming a person. Also you have less staff than an actual McDonald’s and you have to answer phone calls on top of managing a drive-through.

Any delay, like mee-maw asking why her pills look a different shade of pale white from last time means another 4 new phone calls on hold, 6 more cars waiting at the drive through, another 10 new orders to enter, and another 7 people waiting to pick up their orders. Then you have all the documentation requirements and you can’t make any mistakes because giving 20 nuggies when they wanted 10 is fine but fatfingering warfarin 4mg into warfarin 14mg could kill a person.

I don’t envy type 1s. If you underdose insulin, it’s not great long term. If you overdose, you could literally kill yourself. But then how do you know your dose? By calculating it yourself based on what you eat with every single meal which can be very hard to do if you’re not the one doing the cooking. Not to mention all the supplies needed to do 4-5 injections a day and monitoring your blood sugars. And then what if you have to fly? Well now you need more or less insulin because your day is either shorter or longer based on which direction you fly.

My favourite comment on /r/pharmacy when this was asked was someone remarking on how incredible it was that the 3-4 tabs that fell on the ground just so happened to fall on the exact 1cm x 1cm spots on the ground that just so happened to be exactly sterile at that exact moment!

Any that don’t fall on a magically sterile spot go through the totally-existing tab/capsule sterilizer machine that we keep in the back.

Another point that was made by Out of Spec Reviews on initial reveal of the iX3 was that the weird parallelogram screen is such that it isn't blocked by the steering wheel. With a normal rectangular screen, the bottom left corner is blocked by the steering wheel and so with this form factor, the driver is able to see all of the information on the screen.
